The effectiveness of low power laser and occlusal plate in the treatment of temporomandibular dysfunctions: a literature review

Resumo: Temporomandibular dysfunctions (TMD's) represents a set of functional and structural disorders of the stomatognathic system. The main causes of non-dental pain in the orofacial region are considered. Its treatment modalities are diverse and depend essentially on diagnosis. This work aims to clarify the efficacy of low-power laser associated or not with occlusal plaque in the treatment of temporomandibular dysfunctions. Studies published from 2016 to 2022 were selected through the databases: National Library of Medicine (PubMed), Scientific Electronic Library (SciELO) and ScienceDirect. The descriptors employed were: Dysfunction/Tempromandibular Disorders; Occlusal Splint; Low-Power/Low-Level Laser and Boolean and operator between them. We included studies that related low-power laser and occlusal plate individually and jointly as a therapeutic approach for the treatment of temporomandibular dysfunctions, with 16 of which were selected. The literature indicates therapeutic effectiveness for both associates and/or isolates for the treatment of TMDs, but there are controversies about the indication of the two noninvasive therapeutic methods. There is a scarcity of clinical studies that proves the efficacy of these treatments and, also, great heterogeneity of the protocols addressed, making it difficult to parameterize the effectiveness of therapies.

Se consideran las principales causas de dolor no dental en la región orofacial. Sus de tratamiento son diversas y dependen esencialmente del diagnóstico. Este trabajo tiene como objetivo aclarar la eficacia del láser de baja potencia asociado o no con la placa oclusal en el tratamiento de las disfunctions temporomandibulares. Los estudios publicados de 2016 a 2022 fueron seleccionados a través de las bases de datos: Biblioteca Nacional de Medicina (PubMed), Biblioteca Electrónica Científica (SciELO) y ScienceDirect. Los descriptors empleados fueron: Disfunción/Trastornos Tempromandibulares; Férula oclusal; Láser de baja potencia / bajo nivel y booleano y operador entre ellos. Se incluyeron estudios que relacionaban el láser de baja potencia y la placa oclusal de forma individual y conjunta como un enfoque terapéutico para el tratamiento de las disfunctions temporomandibulares, de los cuales se selecton 16. La literatura indica efectividad terapéutica tanto para asociados como para aislados para el tratamiento de TMD, pero existen controversias sobre la indicación de los dos métodos terapéuticos no invasivos. Existe una escasez de studios clínicos que demuestren la eficacia de estos tratamientos y, también, una gran heterogeneidad de los protocolos abordados, lo que dificulta parametrizar la efectividad de las terapias.As Disfunções Temporomandibulares (DTM’s) representam um conjunto de distúrbios funcionais e estruturais do sistema estomatognático. São consideradas as principais causas de dor de origem não dentaria na região orofacial. Suas modalidades de tratamentos são diversas e dependem essencialmente do diagnóstico. Este trabalho tem objetivo de esclarecer a eficácia do laser de baixa potência associado ou não à placa oclusal no tratamento das disfunções temporomandibulares. Foram selecionados estudos publicados no período de 2016 a 2022, através dos bancos de dados: National Library of Medicine (PubMed), Scientific   Electronic   Library (SciELO) e ScienceDirect. Os descritores empregados foram: Dysfunction/Disorders Tempromandibular; Splint Occlusal; Low-Power/Low-Level Laser e operador booleano AND entre eles. Foram incluídos estudos que relacionavam o laser de baixa potência e a placa oclusal, de forma individual e conjunta, como abordagem terapêutica para o tratamento das disfunções temporomandibulares, sendo 16 os artigos selecionados. A literatura aponta efetividade terapêutica para ambos associados e/ou isolados para tratamento das DTM’s, mas há controvérsias sobre a indicação dos dois métodos terapêuticos não invasivos.
