Resumo: | Child care encompasses numerous circumstances within the health process, which include several factors and circumstances to be evaluated. As a result, humanized care is highlighted both for hospitalized children and for health professionals, who need support and follow-up. Children with chronic conditions undergo numerous procedures in the health services until the moment of their discharge, therefore, this procedure has a sequence of care and assistance that must be carried out. Therefore, knowing the relevance of this theme, this study aimed to highlight the management of care that must be performed by health professionals in children with chronic diseases and conditions. For this, the research was carried out through a descriptive and exploratory study, of the integrative literature review type, carried out in the scientific databases LILACS and SCIELO, selecting scientific evidence from the last 5 years to compose the analysis of the results. By analyzing the literature, some important points were highlighted regarding the management of care that must be performed by health professionals in children with chronic diseases and conditions. In this context, the literature highlights that the preparation for hospital discharge involves a complex dynamic and suffers from several difficulties in the health services, including the difficulty of actions that are directly related to the preparation of the discharge carried out by the multidisciplinary team. |

Como resultado, se destaca el cuidado humanizado tanto para los niños hospitalizados como para los profesionales de la salud, que necesitan apoyo y seguimiento. Los niños con condiciones crónicas son sometidos a numerosos procedimientos en los servicios de salud hasta el momento de su alta, por lo tanto, este procedimiento tiene una secuencia de cuidados y asistencia que debe realizarse. Por lo tanto, conociendo la relevancia de este tema, este estudio tuvo como objetivo resaltar la gestión del cuidado que debe ser realizado por los profesionales de la salud en niños con enfermedades y condiciones crónicas. Para ello, la investigación se realizó a través de un estudio descriptivo y exploratorio, del tipo revisión integrativa de la literatura, realizado en las bases de datos científicas LILACS y SCIELO, seleccionando evidencias científicas de los últimos 5 años para componer el análisis de los resultados. Mediante el análisis de la literatura, se destacaron algunos puntos importantes sobre la gestión del cuidado que debe ser realizado por los profesionales de la salud en niños con enfermedades y condiciones crónicas. En ese contexto, la literatura destaca que la preparación para el alta hospitalaria involucra una dinámica compleja y sufre de varias dificultades en los servicios de salud, incluyendo la dificultad de las acciones que están directamente relacionadas con la preparación del alta realizada por el equipo multidisciplinario.O cuidado com a criança, engloba inúmeras circunstâncias dentro do processo de saúde, que englobam diversos fatores e circunstâncias a serem avaliados. Mediante a isso, destaca-se a assistência humanizada tanto a criança hospitalizada como também para os profissionais de saúde, que necessitam de amparo e acompanhamento. A criança com condições crônicas, passam por inúmeros procedimentos nos serviços de saúde até o momento da sua alta, mediante a isso, esse procedimento existe uma sequência de cuidados e assistência que devem ser realizados. Mediante a isso, conhecendo a relevância dessa temática, este estudo objetivou evidenciar o manejo dos cuidados que devem ser realizados pelos profissionais de saúde em crianças com doenças e condições crônicas. Para isso, a pesquisa foi feita através de um estudo descritivo e exploratório, do tipo revisão integrativa da literatura, realizado nas bases de dados científicas LILACS e SCIELO, selecionando evidências científicas dos últimos 5 anos para compor análise dos resultados. Mediante análise da literatura, alguns pontos importantes foram evidenciados no que tange o manejo dos cuidados que devem ser realizados pelos profissionais de saúde em crianças com doenças e condições crônicas.
