Epidemiological profile of sepsis hospitalizations in Brazil between 2017 and 2021

Resumo: The objective was to analyze the epidemiological profile and temporal evolution of hospitalizations, deaths and hospital mortality rate due to sepsis in Brazil between 2017 and 2021. This is an observational, cross-sectional study with a quantitative approach, conducted from the the Hospital Morbidity Information System of the Unified Health System. Data were collected on hospital admissions, deaths and hospital mortality rates for sepsis between 2017 and 2021, according to Brazilian regions, age group, gender, color/race of individuals, and year of care. The results were analyzed using descriptive statistics. A total of 615,805 hospitalizations, 279,765 hospital deaths and a mean hospital mortality rate of 45.49 per 100 hospitalizations per year in Brazil were observed.  Both hospitalizations and deaths were more common in the Southeast, among the elderly over 60 years of age, females and caucasians. The mortality rates were higher in the Southeast region, in the age group of 80 years or older, in females and in self-identified blacks. Observing the sex, there was no expressive numerical difference between females and males and regarding color/race, a question mark was raised regarding the completeness of the data. The temporal evolution of hospital admissions and deaths showed an increase until 2019 and reduction until 2021, with hospital mortality rates decreasing until 2019 and increasing until 2021, a result different from those in the literature. Thus, we emphasize the need for further nationwide studies on this grievance, in addition to public policies for early detection of sepsis in this population.

Se trata de un estudio observacional, transversal y con enfoque cuantitativo, realizado a partir del Sistema de Informação sobre Morbidade Hospitalar do Sistema Único de Saúde. Se recogieron datos sobre los ingresos hospitalarios, las muertes y las tasas de mortalidad hospitalaria por sepsis entre 2017 y 2021, según las regiones brasileñas, el grupo de edad, el género, el color/raza de los individuos y el año de atención. Los resultados se analizaron mediante estadísticas descriptivas. Se observó un total de 615.805 hospitalizaciones, 279.765 muertes hospitalarias y una tasa media de mortalidad hospitalaria de 45,49 por cada 100 hospitalizaciones al año en Brasil.  Tanto en las hospitalizaciones como en las muertes hubo una mayor implicación en la región del sureste, en los mayores de 60 años, en las mujeres y en la raza blanca. Las tasas de mortalidad fueron más altas en la región del sureste, en el grupo de edad de 80 años o más, en las mujeres y en los negros autodenominados. Observando el sexo, no había ninguna diferencia numérica expresiva entre las hembras y los machos, y en cuanto al color/raza, se planteó un interrogante sobre la exhaustividad de los datos. En cuanto a la evolución temporal de los ingresos y las defunciones hospitalarias, se observa un aumento hasta 2019 y una reducción hasta 2021, con una disminución de las tasas de mortalidad hospitalaria hasta 2019 y un aumento hasta 2021, un resultado diferente al de la literatura. Por lo tanto, cabe destacar la necesidad de realizar más estudios nacionales sobre este agravio, además de políticas públicas para la detección temprana de la sepsis en esta población.Objetivou-se analisar o perfil epidemiológico e evolução temporal das internações, óbitos e taxa de mortalidade hospitalar por sepse no Brasil, entre 2017 e 2021. Trata-se de estudo observacional, transversal e de abordagem quantitativa, realizado a partir do Sistema de Informação sobre Morbidade Hospitalar do Sistema Único de Saúde. Foram coletados dados sobre internações, óbitos e taxas de mortalidade hospitalar por sepse entre 2017 e 2021, segundo regiões brasileiras, faixa etária, sexo, cor/raça dos indivíduos e ano de atendimento. Os resultados foram analisados por meio de estatística descritiva. Foi observado um total de 615.805 internações, 279.765 óbitos hospitalares e uma taxa média de mortalidade hospitalar de 45,49 a cada 100 internações por ano no Brasil.  Tanto nas internações quanto nos óbitos houve maior acometimento na região Sudeste, em idosos acima de 60 anos, no sexo feminino e raça branca. As taxas de mortalidade foram maiores na região Sudeste, na faixa etária de 80 anos ou mais, no sexo feminino e em autointitulados pretos. Observado o sexo, não houve diferença numérica expressiva entre o feminino e o masculino e, relativo à cor/raça, levantou-se um questionamento quanto à completitude dos dados. Vista evolução temporal das internações e óbitos hospitalares, observou-se aumento até 2019 e redução até 2021, com taxas de mortalidade hospitalar decrescentes até 2019 e ascendentes até 2021, resultado distinto aos da literatura.
