Aspects involved in the sustainability of the comprehensive medication management services

Resumo: Comprehensive Medication Management (CMM) services produce positive clinical, humanistic and economic results in several scenarios, and studies have already elucidated the components necessary for its implementation. Despite this, most initiatives are still unsuccessful in maintaining the long-term provision of the service. Thus, this study aimed to understand the elements that contribute to the consolidation and continuation of the CMM service. The chosen methodology was grounded theory. In the period from May 2019 to February 2020, semi-structured interviews were conducted with eight pharmacists and four managers who experienced the offering of CMM in various scenarios of public and private healthcare in Belo Horizonte and the metropolitan region. Records in the first author's field diaries and document analysis were also used. After systematic data analysis, as recommended by the grounded theory methodology, three determinant categories were identified in the CMM sustainability process. The first one, called “Collaborative care is essential: You can't think of CMM just like the pharmacist and patient dyad”, which addresses the essentiality of professional partnerships to strengthen the service. The second category, “Negotiating the sustainability of the CMM”, evaluates important elements associated with service management. And the third, “Who will pay for the service?”, analyzes the possibilities of financial resources in health systems. The results revealed that the consolidation of CMM services is a strategic process, which requires careful planning that begins with the implementation of the service.

A pesar de esto, la mayoría de las iniciativas no logran perpetuar la oferta de servicios a largo plazo. Así, este estudio tiene como objetivo comprender los elementos que contribuyen a la consolidación y perpetuación del servicio GIF. La metodología elegida fue la teoría fundamentada en los datos. Desde mayo de 2019 hasta febrero de 2020, se realizaron entrevistas semiestructuradas con 8 farmacéuticos y 4 gerentes que experimentaron la oferta del GIF en diferentes escenarios de salud pública y privada en Belo Horizonte y región metropolitana. También se utilizaron registros en los diarios de campo del investigador y análisis de documentos. Después del análisis sistemático de los datos, según lo recomendado por la teoría fundamentada, se identificaron tres categorías determinantes en el proceso de sustentabilidad del GFT. El primero, titulado "La atención colaborativa es esencial: no se puede pensar en GFT solo como el dúo farmacéutico-paciente", que aborda la esencialidad de trabajar junto a otros profesionales para fortalecer el servicio. La segunda categoría, “Negociar la sostenibilidad de GTM”, evalúa elementos importantes sobre la gestión de servicios. Y el tercero, “¿Quién pagará el servicio?”, analiza las posibilidades de recursos económicos en los sistemas de salud. Los resultados revelaron que la consolidación de los servicios GTM es un proceso estratégico, que requiere una planificación cuidadosa, comenzando con la implementación del servicio.O serviço de gerenciamento da terapia medicamentosa (GTM) apresenta resultados clínicos, humanísticos e econômicos positivos em vários cenários de prática. Estudos já elucidaram os componentes necessários para sua implantação. Apesar disso, a maioria das iniciativas ainda não tem sucesso na perenização da oferta do serviço em longo prazo. Assim, este estudo objetivou compreender elementos que contribuem para a consolidação e perenização do serviço de GTM. A metodologia escolhida foi a teoria fundamentada nos dados. No período de maio de 2019 a fevereiro de 2020, entrevistas semiestruturadas foram conduzidas com oito farmacêuticos e quatro gestores que vivenciaram a oferta do GTM em diversos cenários da saúde pública e privada de Belo Horizonte e região metropolitana. Foram utilizados também registros em diários de campo da pesquisadora e análise de documentos. Após análise sistemática dos dados, como preconizado pela teoria fundamentada, foram identificadas três categorias determinantes no processo de sustentabilidade do GTM. A primeira, denominada “O cuidado colaborativo é essencial: Não dá pra pensar em GTM somente como a díade farmacêutico e paciente”, que aborda a essencialidade das parcerias profissionais para fortalecer o serviço. A segunda categoria, “Negociando a sustentabilidade do GTM”, avalia elementos importantes acerca da gestão do serviço. E a terceira, “Quem vai pagar pelo serviço?”, analisa as possibilidades de recursos nos sistemas de saúde para financiamento do serviço. Os resultados revelaram que a consolidação dos serviços de GTM é um processo estratégico, que exige um planejamento cuidadoso iniciado na implantação do serviço.
