Strengthening patient safety strategies: an integrative review as to drug safety processes

Resumo: | Objectives: To identify evidence regarding the processes used to promote patient safety related to the use of drugs in hospital environments and what implications these actions have for clinical practice and pharmaceutical care. Methods: Integrative review carried out on the databases, Cochrane Library, Epistemonikos, Health Evidence, Health Systems Evidence, and Virtual Health Library, using the domains "elderly", "patient safety", "polypharmacy" combining them with the Boolean operators 'AND' and 'OR'. The publications were selected according to the inclusion and exclusion criteria; the articles were analyzed in pairs; there was no language restriction. Results: The search resulted in 73 articles, of which: 42 were excluded due to different themes to that of this review, 4 systematic reviews, and 3 duplicates. There remained 24 eligible articles divided into 5 thematic axes: perception of professionals and patients regarding safety in the use of medicines; delivery systems and standardized drug list; drug dispensing by pharmacists and pharmacy technicians; stability test; and, quality and management of assistance processes. Conclusion: There are numerous strategies employed to promote safety in the use of medicines and the pharmacist, as a technical and clinical consultant in pharmaceutical care, can act by detecting potential flaws and outlining strategies to promote patient safety in order to reduce costs with health technologies. |

Métodos: Revisión integrativa realizada en las bases de datos, Cochrane Library, Epistemonikos, Health Evidence, Health Systems Evidence y Virtual Health Library, utilizando los dominios "anciano", "seguridad del paciente", "polifarmacia" combinándolos con los operadores booleanos 'Y' y 'O'. Las publicaciones fueron seleccionadas de acuerdo con los criterios de inclusión y exclusión, los artículos fueron analizados por pares; no hubo restricción de idioma. Resultados: La búsqueda resultó en 73 artículos, de los cuales: 42 fueron excluidos por temática diferente a la de esta revisión, 4 revisiones sistemáticas, y 3 duplicados; Quedaron 24 artículos elegibles, divididos en 5 ejes temáticos: percepción de profesionales y pacientes sobre la seguridad en el uso de medicamentos; sistemas de entrega y lista de medicamentos estandarizada; dispensación de medicamentos por farmacéuticos y técnicos de farmacia; Examen de estabilidad; y calidad y gestión de los procesos de asistencia. Conclusión: Existen numerosas estrategias empleadas para promover la seguridad en el uso de medicamentos y el farmacéutico, como consultor técnico y clínico en la atención farmacéutica, puede actuar detectando posibles fallas y delineando estrategias para promover la seguridad del paciente con el fin de reducir costos con tecnologías sanitarias.Objetivos: Identificar evidências quanto aos processos utilizados para a promoção da segurança do paciente relacionados à utilização de medicamentos em ambientes hospitalares e quais implicações dessas ações para a prática clínica e do cuidado farmacêutico. Métodos: Revisão integrativa realizada nas bases de dados, Cochrane Library, Epistemonikos, Health Evidence, Health Systems Evidence, e Biblioteca Virtual de Saúde, utilizando-se os domínios “idosos”, “segurança do paciente”, “polifarmácia” combinando-os com os operadores booleanos 'AND' e 'OR'. As publicações foram selecionadas de acordo com os critérios de inclusão e exclusão; a análise dos artigos se deu por pares; não houve restrição de idiomas. Resultados: A busca resultou em 73 artigos, dos quais: 42 foram excluídos por temática diversa à desta revisão, 4 revisões sistemáticas, e 3 duplicatas. Restaram 24 artigos elegíveis divididos em 5 eixos temáticos: percepção dos profissionais e dos pacientes quanto à segurança no uso de medicamentos; sistemas de liberação e relação de medicamentos padronizados; dispensação de medicamentos realizada por farmacêuticos e técnicos de farmácia; teste de estabilidade; e, qualidade e gestão de processos assistenciais.
