Analysis of factors associated with adherence to drug treatment of patients with type 2 Diabetes mellitus in family health strategy units

Resumo: Diabetes Mellitus is a chronic disease quite prevalent in the Brazilian population. Non-adherence to treatment implies serious complications for the patient, such as amputations and blindness. The objective of the present study was to identify the prevalence of adherence to drug treatment and the factors associated with non-adherence in patients diagnosed with type 2 Diabetes Mellitus (DM2), users of Family Health Strategies located in the city of Junqueirópolis, São Paulo. The sample consisted of patients, of both sexes, registered in the health system of Junqueirópolis, randomly drawn in each region of the city, making a total of 489 participants. The sociodemographic profile, clinical and subjective variables related to the treatment were addressed through a structured interview using a closed questionnaire. The Treatment Adherence Measure (MAT) was used to assess the individual's behavior in relation to the daily use of medication. Categorical variables are presented by their absolute and relative values, comparisons with independent variables were performed using the Chi-square test, adjusted for other confounding variables (gender, age). The results obtained show a high rate of adherence to drug treatment in the interviewed patients with DM2. There was a statistically significant association between the subjective variables presented and adherence to treatment, including having knowledge of the disease and regular monitoring of the condition, a fact that demonstrates the importance of the educational role of primary care in controlling the pathology.

La no adherencia al tratamiento implica graves complicaciones para el paciente, como amputaciones y ceguera. El objetivo del presente estudio fue identificar la prevalencia de adherencia al tratamiento farmacológico y los factores asociados a la no adherencia en pacientes diagnosticados con Diabetes Mellitus tipo 2 (DM2), usuarios de Estrategias de Salud de la Familia ubicados en el municipio de Junqueirópolis, São Paulo. La muestra estuvo compuesta por pacientes, de ambos sexos, registrados en el sistema de salud de Junqueirópolis, seleccionados al azar en cada región de la ciudad, haciendo un total de 489 participantes. El perfil sociodemográfico, las variables clínicas y subjetivas relacionadas con el tratamiento se abordaron a través de una entrevista estructurada mediante un cuestionario cerrado. La Medida de Adherencia al Tratamiento (MAT) fue utilizada para evaluar el comportamiento del individuo en relación al uso diario de medicamentos. Las variables categóricas se presentan por sus valores absolutos y relativos, las comparaciones con las variables independientes se realizaron mediante la prueba de Chi-cuadrado, ajustada por otras variables de confusión (género, edad). Los resultados obtenidos muestran una alta tasa de adherencia al tratamiento farmacológico en los pacientes con DM2 entrevistados. Hubo asociación estadísticamente significativa entre las variables subjetivas presentadas y la adherencia al tratamiento, incluyendo el conocimiento de la enfermedad y el seguimiento regular de la condición, hecho que demuestra la importancia del papel educativo de la atención primaria en el control de la patología.O Diabetes Mellitus é uma doença crônica bastante prevalente na população brasileira. O objetivo do presente estudo foi identificar a prevalência da adesão ao tratamento medicamentoso e os fatores associados à não adesão em pacientes diagnosticados com Diabetes Mellitus tipo 2 (DM2), usuários de Estratégias de Saúde da Família situadas na cidade de Junqueirópolis, São Paulo. A amostra foi composta por pacientes, de ambos os sexos, cadastrados no sistema de saúde de Junqueirópolis, sorteados aleatoriamente em cada região da cidade, perfazendo o total de 489 participantes. O perfil sociodemográfico, variáveis clínicas e subjetivas relacionadas ao tratamento foram abordados por meio de entrevista estruturada utilizando-se um questionário fechado. A Medida de Adesão aos Tratamento (MAT) foi empregada para avaliar o comportamento do indivíduo em relação ao uso diário dos medicamentos. As variáveis categóricas são apresentadas pelos seus valores absolutos e relativos, as comparações com variáveis independentes foram realizadas utilizando-se o teste de Qui quadrado, ajustada por demais variáveis de confusão (sexo, idade). Os resultados obtidos mostram alta taxa de adesão ao tratamento medicamentoso nos pacientes com DM2 entrevistados.
