Resumo: Third molar extractions are common in routine dentistry, especially among clinicians and maxillofacial surgeons. However, like any surgery, they are also susceptible to accidents and complications during and after surgery. Factors of advanced age, systemic diseases of age, impaction and dental-surgical procedure, directly not success of the procedure operation. This study aimed to explore accidents and complications related to lower third molar surgery. Thus, this study presents itself as a narrative literature review, through a qualitative approach, through the understanding of the proposed theme. Therefore, the databases used were Pubmed, Google Scholar and Lilacs. From the study on the subject through 20 literature review studies and case reports. It can be seen that the main complications and accidents were alveolitis, hemorrhages, nerve injuries and fractures. Recurrent pericoronitis is the most common reason for third molar extraction, as it relates to impacted teeth mesioangulated and vertical. Alveolitis, hemorrhages, fractures and nerve solutions are related to most third-party complications, but not routine, in short, the importance of the authors emphasize has no dominion over the situation, whether in prevention or not hand-to-hand . Therefore, it is necessary to emphasize the importance of adequate and individualized planning, because in this way it is possible to prevent accidents and understand the correct management of complications.

Sin embargo, como cualquier cirugía, también son susceptibles a accidentes y complicaciones durante y después de la cirugía. Factores como la edad avanzada, las enfermedades sistémicas, el grado de impactación y la experiencia del odontólogo influyen directamente en el éxito del procedimiento quirúrgico. Este estudio tuvo como objetivo explorar los accidentes y las complicaciones relacionadas con la cirugía del tercer molar inferior. Así, este estudio se presenta como una revisión narrativa de la literatura, a través de un abordaje cualitativo, a través de la percepción del tema propuesto. Por lo tanto, las bases de datos utilizadas fueron Pubmed, Google Scholar y Lilacs. A partir del estudio sobre el tema a través de 20 estudios de revisión bibliográfica y reportes de casos. Se puede observar que los principales accidentes y complicaciones fueron alveolitis, hemorragias, lesiones nerviosas y fracturas. La pericoronitis recurrente es la razón más común para la extracción del tercer molar, ya que se relaciona con dientes mesioangulados e impactados verticalmente. Las alveolitis, las hemorragias, las fracturas y las lesiones nerviosas son mayoritarias en lo que respecta a las complicaciones de los terceros molares en el acto de la escisión, pero no de rutina, en fin, la importancia que destacan los autores está en el dominio sobre la situación, ya sea en la prevención o en la el manejo de las complicaciones. Por lo tanto, es necesario enfatizar la importancia de una planificación adecuada e individualizada, pues de esta forma es posible prevenir accidentes y comprender el manejo correcto de las complicaciones.Extrações de terceiro molar são comuns na rotina odontológica, principalmente entre clínicos e cirurgiões bucomaxilofaciais. Entretanto, como qualquer cirurgia, estas também estão susceptíveis a acidentes e complicações no trans-operatório e pós-operatório. Fatores como idade avançada, doenças sistêmicas, grau de impacção e experiência do cirurgião-dentista influenciam diretamente no sucesso do procedimento cirúrgico. Este trabalho objetivou explorar acerca dos acidentes e complicações relacionados a cirurgia de terceiro molar inferior. Assim, este estudo apresenta-se como uma revisão de literatura narrativa, por meio de uma abordagem qualitativa, através da percepção do tema proposto. Logo, as bases de dados utilizadas foram Pubmed, Google Acadêmico e Lilacs. A partir do estudo acerca da temática através de 21 estudos de revisão de literatura e relatos de casos. Pode-se evidenciar que os principais acidentes e complicações foram alveolite, hemorragias, lesões nervosas e fraturas. A pericoronarite recorrente é o motivo mais comum para exodontia de terceiros molares, pois faz uma relação com dentes impactados mesioangulados e verticais. A alveolite, hemorragias, fraturas e lesões nervosas estão em maioria com relação as complicações de terceiros molares no ato de exérese, mas não rotineiras, em suma disso, a importância que os autores enfatizam está no domínio sobre a situação, seja na prevenção como no manejo da complicação.
