Benefits of using honey to treat diabetic foot: A scoping review

Resumo: Diabetic foot can be understood as the destruction of soft tissues, and as infections or ulcerations associated with neurological and vascular changes in the lower limbs of patients with Diabetes Mellitus (DM). It is also one of the most severe, costly, and devastating complications of DM, and foot ulcers are precursors of lower limb amputations in 85% of the cases. Thus, honey emerges as an alternative treatment which is economical, presents a low risk of bacterial resistance, and is widely available, indispensable factors for the continuity of treatment by the patients. This article is a scoping review with the objective of describing the evidence to elucidate the benefits of honey in the treatment of diabetic foot. The research includes primary studies, reviews, meta-analyses and/or meta-syntheses, books and guidelines, theses, and dissertations, published in indexed sources or in the gray literature published in the following databases: Scielo, LILACS, Pubmed, The Cochrane Library, Web of Science, SCOPUS, CINAHL, and Academic Search Premier, CAPS, the USP Digital Library of Theses and Dissertations, and the USP Integrated Search Portal. In the literature reviewed, 33 different benefits have been found which are described for the use of honey. Among these benefits, the higher healing rate and anti-microbial, anti-inflammatory, and debriding properties stand out, among others. Therefore, the analyzed studies showed a wide variety of benefits, which reflects the complexity of the action and the usefulness of this product.

Es también una de las complicaciones más graves, costosas y devastadoras de la DM, y las úlceras del pie son en el 85% de los casos, precursoras de la amputación de miembros inferiores. Así, la miel surge como un tratamiento alternativo, que es económico, presenta poco riesgo de resistencia bacteriana y se encuentra ampliamente disponible, factores imprescindibles para la continuidad del tratamiento por parte de los pacientes. Este artículo es una revisión de alcance que tiene como objetivo describir la evidencia para dilucidar los beneficios de la miel en el tratamiento del pie diabético. La investigación incluye estudios primarios, revisiones, metanálisis y/o meta-síntesis, libros y guías, tesis y disertaciones, publicados en fuentes indexadas o en la literatura gris publicada en las siguientes bases de datos: Scielo, LILACS, Pubmed, The Cochrane Library, Web of Science, SCOPUS, CINAHL y Academic Search Premier, CAPS, Biblioteca Digital de Tesis y Disertaciones de la USP, y Portal de Búsqueda Integrada de la USP. En la literatura revisada, se han encontrado 33 beneficios diferentes descriptos para el uso de la miel. Entre ellos, se destacan la mayor tasa de curación y propiedades antimicrobianas, antiinflamatorias y de desbridamiento, entre otras. Por lo tanto, los estudios analizados demostraron una amplia variedad de beneficios, lo que refleja la complejidad de la acción y la utilidad de este producto.O pé diabético pode ser compreendido como destruição de tecidos moles, infecção ou ulceração associada a alterações neurológicas e vasculares nos membros inferiores de portadores de diabetes mellitus (DM). É também uma das mais graves, dispendiosas e devastadoras complicações da DM, sendo as úlceras nos pés precursoras, em 85% dos casos, da amputação do membro inferior. Assim, o mel surge como uma alternativa de tratamento, sendo barato, com pouco risco de resistência bacteriana e amplamente disponível, fatores imprescindíveis para a continuidade do tratamento pelos pacientes. Este artigo é uma Scoping Rewiew que pretende descrever as evidências de forma a elucidar os benefícios do mel no tratamento do pé diabético. A pesquisa engloba estudos primários, revisões, metanálises e/ou metassínteses, livros e guidelines, teses e dissertações, publicados em fontes indexadas ou na literatura cinzenta publicados nas bases de dados: Scielo, LILACS, Pubmed, The Cochrane Library, Web of Science, SCOPUS, CINAHL e Academic Search Premier, CAPS, Biblioteca Digital de Teses e Dissertações USP, Busca Integrada USP. Na literatura revisada, foram encontrados 33 diferentes benefícios descritos para o uso do mel.  Dentre eles, destacam-se a maior taxa de cicatrização, propriedades antimicrobianas, anti-inflamatórias e desbridantes, entre outras.
