Palliative care and bone marrow transplantation: an integrative literature review

Resumo: Introduction: Bone Marrow Transplantation (BMT) is a treatment with important curative potential, which raises great hope in patients experiencing diseases that pose a serious threat to the continuity of life. However, such treatment is permeated by high risks, in addition to the possibility of relapse of the disease still existing after it, thus, it is important to consider care approaches that cover this unpredictability, such as Palliative Care. Objective: To describe the contributions of the palliative care approach in bone marrow transplantation, which is characterized as a treatment with a curative proposal for patients with diseases that pose a serious threat to the continuity of life. Method: Integrative Literature Review carried out in the Scopus, PubMed, PsychINFO and LILACS databases, covering studies from 2011 to 2021. The studies were categorized based on content analysis. Results: After applying the exclusion criteria, six studies remained that made up the corpus of this work. Results showed that the early integration of palliative care into standard BMT care significantly contributes to the management of physical and emotional symptoms, exploration of topics not addressed by other professionals, such as coping with the moment, as well as discussions about advance care planning. Conclusion: Palliative Care adds numerous gains in relation to the care of the patient undergoing BMT. However, there is a scarcity of studies involving both themes and the need to develop new studies that can promote this practice within BMT clinics.

Sin embargo, tal tratamiento está permeado por altos riesgos, además de la posibilidad de recaída de la enfermedad aún existente después del mismo, por lo que es importante considerar abordajes asistenciales que abarquen esa imprevisibilidad, como los cuidados caliativos. Objetivo: Describir los aportes del enfoque de cuidados paliativos en el trasplante de médula ósea, el cual se caracteriza por ser un tratamiento con propuesta curativa para pacientes con enfermedades que representan una grave amenaza para la continuidad de la vida. Método: Revisión Integrativa de Literatura realizada en las bases de datos Scopus, PubMed, PsychINFO y LILACS, abarcando estudios desde 2011 hasta 2021. Los estudios fueron categorizados a partir del análisis de contenido. Resultados: Después de aplicar los criterios de exclusión, quedaron seis estudios que integraron el corpus de este trabajo. Los resultados mostraron que la integración temprana de los cuidados paliativos en la atención estándar de TMO contribuye significativamente al manejo de los síntomas físicos y emocionales, la exploración de temas no abordados por otros profesionales, como el afrontamiento del momento, así como las discusiones sobre la planificación anticipada de la atención. Conclusión: Los cuidados caliativos añaden numerosas ganancias en relación al cuidado del paciente sometido a TMO. Sin embargo, existe una escasez de estudios que involucren ambos temas y la necesidad de desarrollar nuevos estudios que puedan promover esta práctica dentro de las clínicas de TMO.Introdução: O Transplante de Medula Óssea (TMO) consiste em um tratamento de importante potencial curativo, o que desperta grande esperança em pacientes vivenciando doenças que exercem grave ameaça à continuidade da vida. Entretanto, tal tratamento é permeado por altos riscos, além da possibilidade de recaída da doença ainda existir após o mesmo, dessa forma, faz-se importante considerar abordagens de cuidado que abarquem essa imprevisibilidade, como os Cuidados Paliativos. Objetivo: Descrever quais as contribuições da abordagem de cuidados paliativos no transplante de medula óssea, que se caracteriza como um tratamento de proposta curativa a pacientes com doenças que exercem grave ameaça à continuidade da vida. Método: Revisão Integrativa da Literatura realizada nas bases de dados Scopus, PubMed, PsychINFO e LILACS, abarcando estudos de 2011 a 2021. Os estudos foram categorizados a partir da análise de conteúdo. Resultados: Após aplicação dos critérios de exclusão restaram seis estudos que compuseram o corpus deste trabalho. Resultados demonstraram que a integração precoce de cuidados paliativos ao cuidado padrão de TMO contribui significativamente para o manejo de sintomas físicos e emocionais, exploração de temáticas não abordadas por outros profissionais, como o enfrentamento do momento, bem como discussões sobre planejamento antecipado de cuidado. Conclusão: Os Cuidados Paliativos agregam inúmeros ganhos em relação ao cuidado do paciente submetido ao TMO.
