Use of laser therapy in the postoperative period of bilateral sagittal split of the mandibular ramus: literature review

Resumo: | Introduction: The most common complication resulting from bilateral sagittal split osteotomy in orthognathic surgery to reposition the mandible in relation to the skull base is the possible damage to the lower alveolar nerve, causing paresthesia in the region of the lower lip and tip of the chin. The incidence of paresthesia after performing this technique is 85-87% of cases. Throughout history, many treatments aimed at restoring sensorineural function in this area have been reported. One of the fields that is currently growing is low power laser therapy. Objective: The aim of this study was to review the literature on the use of laser therapy in the postoperative period of bilateral sagittal osteotomies of the mandibular branch. Methodology: The articles were selected using the PubMed database. For inclusion criteria of the article it should be published in English and whose adopted methodology would allow obtaining significant evidence. Results: The main results found were that low-level laser therapy is effective in helping to control postoperative pain as well as speeding up the repair and regeneration process of tissues, especially nerves, thus reducing postoperative paresthesia. Conclusion: It is concluded that low-power laser therapy is beneficial in accelerating tissue repairs, bringing greater comfort to the patient. However, further studies and follow-up are needed in this area in order to establish a complete application protocol. |

A incidencia de parestesia después de realizar esta técnica es del 85-87% de los casos. A lo largo de la historia, se han reportado muchos tratamientos destinados a restaurar la función neurosensorial en esta área. Uno de los campos que actualmente está creciendo es la terapia con láser de baja potencia. Objetivo: El objetivo de este estudio fue revisar la literatura sobre el uso de la terapia con láser en el postoperatorio de osteotomías sagitales bilaterales de la rama mandibular. Metodología: Los artículos fueron seleccionados utilizando la base de datos PubMed. Para los criterios de inclusión del artículo, debe publicarse en inglés y cuya metodología adoptada permitiría obtener evidencia significativa. Resultados: Los principales resultados encontrados fueron que la terapia con láser de bajo nivel es efectiva para ayudar a controlar el dolor postoperatorio, así como para acelerar el proceso de reparación y regeneración de los tejidos, especialmente los nervios, reduciendo así la parestesia postoperatoria. Conclusión: Se concluye que la terapia con láser de baja potencia es beneficiosa para acelerar las reparaciones de tejidos, brindando mayor comodidad al paciente. Sin embargo, se necesitan más estudios y seguimiento en esta área para establecer un protocolo de aplicación completo.Introdução: A complicação mais comum decorrente da osteotomia sagital bilateral do ramo mandibular em cirurgias ortognáticas para reposicionamento da mandíbula em relação à base do crânio é o possível dano ao nervo alveolar inferior, ocasionando uma parestesia na região de lábio inferior e ponta do queixo. A incidência de parestesia após a realização dessa técnica é de 85-87% dos casos. Ao longo da história muitos tratamentos que visam restaurar a função neurossensorial dessa área foram relatados. Um dos campos que vem crescendo atualmente é a terapia por laser de baixa potência. Objetivo: O objetivo desse estudo foi fazer uma revisão de literatura sobre o uso da laserterapia no pós-operatório de osteotomias sagitais bilaterais do ramo mandibular. Metodologia: Os artigos foram selecionados a utilizando a base de dados da plataforma PubMed. Para critério de inclusão do artigo ele deveria estar publicado em inglês e cuja metodologia adotada permitisse obter evidências significativas. Resultados: Os principais resultados encontrados foram que a terapia com laser de baixa potência é eficaz no auxílio do controle da dor pós-operatória bem como na aceleração do processo de reparo e regeneração dos tecidos, principalmente nervosos, diminuindo assim a parestesia pós-operatória. Conclusão: Conclui-se que a terapia com laser de baixa potência é benéfica na aceleração dos reparos teciduais, levando maior conforto ao paciente.
