Extensive subcutaneous emphysema after exodonty of molar third parties: Case report

Resumo: | Introduction: Subcutaneous emphysema is an accident where the forced introduction of air into the tissues occurs, which in dentistry can appear after the use of a high-speed pen or triple syringe. The swelling associated with emphysema can be mistaken for bruising, allergies or infections. The differential diagnosis is often related to the presence of crackles on palpation, associated or not, with pain. Treatment is prophylactic and palliative, since the problem tends to resolve spontaneously. Objective: To report a clinical case of subcutaneous emphysema and extraction of third molars, discussing the importance of differential diagnosis to guide the treatment of this complication. Case report: A 22-year-old female patient attended the oral and maxillofacial surgery service of a public hospital in Recife-PE, complaining of pain in the lower left third molar region. During the surgical procedure, it was necessary to use a high-speed pen to perform osteotomy and odontosection of tooth 38. At the end of the surgery, there was an increase in volume in the left periorbital region, compatible with subcutaneous emphysema. After 7 days postoperatively, spontaneous resolution of emphysema was observed. Discussion: According to the current literature, after the emphysema is already under a dermal layer, the air may remain at the surgery site or continue to spread along the facial spaces, depending on the permanence of the air jet. Conclusion: The correct diagnosis of subcutaneous emphysema was effective in ruling out other complications, guiding the management of the reported case, in addition to preventing the emphysema from evolving to an infection involving the cervicofacial or thoracic region. |

La hinchazón asociada con el enfisema se puede confundir con hematomas, alergias o infecciones. El diagnóstico diferencial suele relacionarse con la presencia de crepitantes a la palpación, asociados o no, al dolor. El tratamiento es profiláctico y paliativo, ya que el problema tiende a resolverse espontáneamente. Objetivo: Reportar un caso clínico de enfisema subcutáneo posterior a la extracción de terceros molares, discutiendo la importancia del correcto diagnóstico diferencial para orientar el tratamiento de esta complicación. Caso clínico: paciente de 22 años que acude al servicio de cirugía oromaxilofaciales de un hospital público de Recife-PE, por dolor en la región del tercer molar inferior izquierdo. Durante el procedimiento quirúrgico fue necesario utilizar una turbina odontológica para realizar la osteotomía y la odontosección del diente 38. Al final de la cirugía, hubo un aumento de volumen en la región periorbitaria izquierda, compatible con enfisema subcutáneo. A los 7 días del postoperatorio se observó una resolución espontánea del enfisema. Discusión: Según la literatura actual, después de que el enfisema ya está bajo una capa dérmica, el aire puede permanecer en el sitio de la cirugía o continuar extendiéndose por los espacios faciales, dependiendo de la permanencia del chorro de aire. Conclusión: El diagnóstico correcto de enfisema subcutáneo fue efectivo para descartar otras complicaciones, guiando el manejo del caso reportado, además de evitar que el enfisema evolucionara a una infección que afectara la región cervicofacial o torácica.Introdução: O enfisema subcutâneo é um acidente onde ocorre a introdução forçada de ar para o interior dos tecidos, que na odontologia, pode surgir após o uso de caneta de alta rotação ou seringa tríplice. O aumento de volume associado ao enfisema pode ser confundido com hematomas, alergias ou infecções. O diagnóstico diferencial, por muitas vezes, é relacionado à presença de crepitações à palpação, associadas ou não, a dor. O tratamento se dá de forma profilática e paliativa, uma vez que o problema tende a se resolver de forma espontânea. Objetivo: Relatar um caso clínico de enfisema subcutâneo após a exodontia de terceiros molares, discutindo a importância do correto diagnóstico diferencial para guiar o tratamento desta complicação. Relato de caso: Paciente do sexo feminino, 22 anos, compareceu ao serviço de cirurgia buco-maxilo-facial de um hospital público de Recife-PE, queixando-se de dor em região de terceiro molar inferior esquerdo. Durante o procedimento cirúrgico foi necessário o uso de caneta de alta rotação para realizar a ostectomia e odontosecção do dente 38. Ao fim da cirurgia, observou-se um aumento de volume em região periorbital esquerda, compatível com enfisema subcutâneo. Após 7 dias de pós-operatório, observou-se a resolução espontânea do enfisema. Discussão: De acordo com literatura atual, após o enfisema já se encontrar sob a camada dérmica, pode o ar permanecer no local da cirurgia ou continuar se espalhando ao longo dos espaços faciais, a depender da permanência do jato de ar.
