Adherence to treatment of hemodialysis patients with chronic kidney disease: sociodemographic and clinical characterization, facilities and difficulties

Resumo: The objective was to describe the characteristics of patients with chronic kidney disease undergoing hemodialysis, their facilities and difficulties in adhering to treatment. Descriptive study developed with the application of a form with 130 patients from a Ceará hemodialysis clinic, in 2017. Most patients were male (58%), 70% (91) were under 60 years old, 51 % (66) were married and 28% (36) were illiterate. Clinical-epidemiological data showed a greater impact of systemic arterial hypertension and associated diabetes mellitus (37%; 48), followed by only hypertension (28%; 37) and only diabetes (26%; 34). Facilities for adherence to treatment were described as the location of the clinic (40%) and the availability of transportation (12%). As difficulties, adaptation (34%), session time (24%) and access (91%) stood out. However, 80% never thought about abandoning treatment and 91.5% did not report negative thoughts about it, with 96.3% considering themselves happy, with faith and family support. The findings reinforce the patients adaptive capacity of patients to treatment and the need for professionals to strengthen the potential and difficulties of patients, to be worked on by the multidisciplinary team.

Estudio descriptivo desarrollado com la aplicación de um formulário con 130 pacientes de una clínica de hemodiálisis de Ceará, en 2017. La mayoría de los pacientes eran hombres (58%), 70% (91) tenían menos de 60 años, 51% (66 ) estaban casados ​​y el 28% (36) eran analfabetos. Los datos clínico-epidemiológicos mostraron un mayor impacto de la hipertensión arterial sistémica y la diabetes mellitus (DM) asociada (37%; 48), seguida de solo hipertensión (28%; 37) y solo diabetes (26%; 34). Las instalaciones para el cumplimiento del tratamiento se describieron como la ubicación de la clínica (40%) y la disponibilidad de transporte (12%). A maioria dos pacientes era do sexo masculino (58%), 91 (70%) tinham menos de 60 anos, 66 (51%) eram casados e 36 (28%) analfabetos. Nos dados clínico-epidemiológicos, adesão apresentaram maior impacto hipertensão arterial sistêmica e diabetes mellitus associadas (37%; 48), seguido de somente hipertensão (28%; 37) e somente diabetes (26%; 34). Foram descritas como facilidades para adesão ao tratamento a localização da clínica (40%) e a disponibilização de transporte (12%). Como dificuldades destacaram-se a adaptação (34%), o tempo das sessões (24%) e o acesso (91%). No entanto, 80% nunca pensaram em abandonar o tratamento e 91,5% não referem pensamentos negativos sobre ele, sendo que 96,3% se consideram felizes, com fé e apoio familiar.
