Practices of humanized normal birth care: the role of doulas in reducing obstetric violence

Resumo: Objective: To evaluate the perception and practical experiences of Doulas on obstetric violence and the main intervention actions to reduce it. Methods: Qualitative study, with 11 Doulas, identified by D1 to D11 and asked with questions that referred to perceptions of knowledge about obstetric violence, which practices are evidenced throughout the activity, which practices they believe have more impact on reducing violence obstetric and what attitudes were taken to try to reduce them, in addition to the reception of the multidisciplinary health team regarding the intervention of Doula at the time of delivery. The results were analyzed according to the principles of qualitative assessment. Results: The work was developed along 3 axes: obstetric violence in the daily practices of the doulas, effective actions to reduce obstetric violence and the multidisciplinary team's receptivity to the Doulas' interventions. According to the forms, all the doulas show obstetric violence, with episiotomy, Kristeller's maneuver, verbal violence and repetitive vaginal touches being the most cited in the study. As for the receptivity of the team, it was evidenced that it suffers interference from the lack of knowledge about the exact function of doulas, overlapping with other professionals. The fundamental instrument in combating obstetric violence is to provide pregnant women with information about childbirth. Conclusion: The work showed the importance of Doulas associated with obstetric physiotherapy practices as an agent in the humanization of childbirth and the reduction of obstetric violence, requiring further studies with this focus group, aiming at the importance of the topic and its literary scarcity.

Métodos: Estudio cualitativo, con 11 Doulas, identificadas por D1 a D11 y formuladas con preguntas que se referían a percepciones de conocimiento sobre violencia obstétrica, qué prácticas se evidencian a lo largo de la actividad, qué prácticas se cree que tienen mayor impacto en la reducción de la violencia obstétrica y qué actitudes se tomaron para tratar de reducirlas, además de la recepción del equipo multidisciplinario de salud respecto a la intervención de Doula en el momento del parto. Los resultados se analizaron de acuerdo con los principios de la evaluación cualitativa. Resultados: El trabajo se desarrolló en 3 ejes: violencia obstétrica en las prácticas diarias de las doulas, acciones efectivas para reducir la violencia obstétrica y receptividad del equipo multidisciplinario a las intervenciones de las doulas. Según las formas, todas las doulas presentan violencia obstétrica, siendo la episiotomía, maniobra de Kristeller, violencia verbal y toques vaginales repetitivos los más citados en el estudio. En cuanto a la receptividad del equipo, se evidenció que sufre interferencias por el desconocimiento sobre la función exacta de las doulas, solapando con otros profesionales. El instrumento fundamental para combatir la violencia obstétrica es brindar información sobre el parto a las mujeres embarazadas. Conclusión: El trabajo mostró la importancia de las Doulas asociadas a las prácticas de fisioterapia obstétrica como agente en la humanización del parto y la reducción de la violencia obstétrica, requiriendo estudios adicionales con este grupo focal, apuntando a la importancia del tema y su escasez literaria.Objetivo: Avaliar a percepção e vivências práticas das Doulas sobre a violência obstétrica e as principais ações intervencionistas para reduzi-la. Métodos: Estudo de qualitativo, com 11 Doulas, identificadas por D1 a D11 e questionadas com perguntas que remeteram as percepções sobre o conhecimento acerca da violência obstétrica, quais práticas são evidenciadas ao longo da atividade, quais práticas julgam causar mais impacto na redução da violência obstétrica e quais as atitudes tomadas no sentido de tentar reduzi-las, além da recepção da equipe multidisciplinar de saúde quanto a intervenção da Doula no momento do parto. Os resultados foram analisados de acordo com os princípios da avaliação qualitativa. Resultados: O trabalho foi desenvolvido em 3 eixos: violências obstétricas nas práticas diárias das doulas, ações efetivas na redução de violência obstétrica e receptividade da equipe multidisciplinar com as intervenções das Doulas. Segundo os formulários, em sua totalidade as doulas evidenciam violências obstétricas, sendo episiotomia, manobra de Kristeller, violência verbal e toques vaginais repetitivos, as mais citadas no estudo. Quanto a receptividade da equipe, foi evidenciado que sofre interferência da falta de conhecimento sobre a função exata das doulas, sobrepondo a outros profissionais. O instrumento fundamental no combate a violência obstétrica é munir a gestante de informações a cerca do parto.
