Main clinical evidence of hyperbaric oxygen therapy in the treatment of diabetic foot ulcers: a systematic review

Resumo: | Objective: To analyze the main clinical evidence about the use of hyperbaric oxygen therapy in the treatment of diabetic foot ulcers. Methodology: This is a systematic review carried out using the PRISMA method, in the electronic databases PubMed, Scopus and Web of Science, using the following descriptors: “hyperbaric oxygenation”, “diabetes”, “diabetic foot”, “leg ulcer” and “ischemia” Full texts were included, available in Portuguese, English or Spanish, published between January 2016 and December 2021; studies needed to assess the proportion of people with completely healed diabetic foot ulcers in participants randomized to hyperbaric oxygen therapy and in a control group. Results: 08 articles made up the final sample of this study, with a higher frequency of publication in 2019 (n: 03). All studies used the Randomized Clinical Trial methodology, presenting a strong level of evidence (II). Overall, it was found that the rate of amputation in patients undergoing hyperbaric oxygen therapy was lower compared to the standard treatment established in clinical trials. Three studies reported a reduction in ulcer area and five studies showed that hyperbaric oxygen therapy increased the healing rate of diabetic foot ulcers compared to conventional treatment, with statistically significant results. Conclusion: Current evidence shows some evidence of the effectiveness of hyperbaric oxygen therapy in improving the healing of diabetic foot ulcers. Larger and higher quality clinical trials are needed before the implementation of this therapy in routine clinical practice can be justified. |

Se incluyeron textos completos, disponibles en portugués, inglés o español, publicados entre enero de 2016 y diciembre de 2021; se necesitaban estudios para evaluar la proporción de personas con úlceras del pie diabético completamente curadas en los participantes asignados al azar a la oxigenoterapia hiperbárica y en un grupo control. Resultados: 08 artículos conformaron la muestra final de este estudio, con mayor frecuencia de publicación en 2019 (n: 03). Todos los estudios utilizaron la metodología de Ensayo Clínico Aleatorizado, presentando un fuerte nivel de evidencia (II). En general, se encontró que la tasa de amputación en pacientes sometidos a oxigenoterapia hiperbárica fue menor en comparación con el tratamiento estándar establecido en los ensayos clínicos. Tres estudios informaron una reducción en el área de la úlcera y cinco estudios mostraron que la oxigenoterapia hiperbárica aumentó la tasa de cicatrización de las úlceras del pie diabético en comparación con el tratamiento convencional, con resultados estadísticamente significativos. Conclusión: La evidencia actual muestra alguna evidencia de la efectividad de la oxigenoterapia hiperbárica para mejorar la cicatrización de las úlceras del pie diabético. Se necesitan ensayos clínicos más grandes y de mayor calidad antes de que pueda justificarse la implementación de esta terapia en la práctica clínica habitual.Objetivo: Analisar as principais evidências clínicas acerca do uso da oxigenoterapia hiperbárica no tratamento de úlceras de pé diabético. Resultados: 08 artigos compuseram a amostra final desse estudo, com maior frequência de publicação em 2019 (n: 03). Todas as pesquisas apresentaram a metodologia do Ensaio Clínico Randomizado, apresentando nível de evidência forte (II). No geral, identificou-se que a taxa de amputação nos pacientes submetidos a oxigenoterapia hiperbárica foi inferior em relação ao tratamento padrão estabelecido nos ensaios clínicos.
