Assessment of the Pittsburgh Sleep Quality Index in Medical students: An integrative literature review

Resumo: Sleep is an essential biological function of human beings related to quality of life, which can be assessed using scales such as the “Pittsburgh Sleep Quality Index”. Medical students are a population susceptible to poor sleep due to stress, academic obligations and other factors. Thus, the objective of this work was to gather information from the literature on the subject, in view of the growing relevance of the subject in the field of public health. This is an Integrative Literature Review that used descriptors in DeCS and the SciELO, BVS, LILACS and MEDLINE databases. Only studies that used the PSQI index were considered. The search in the databases initially resulted in 140 articles, after applying the exclusion and inclusion criteria, 18 articles were suitable for the final sample. Most articles (70%) showed poor sleep quality among students, but less than 15% had a sleep disorder. The mean of the available PSQI indices was 6.24 and the biggest factor associated with poor sleep quality were depression, anxiety, stress and higher scores for students with Irritable Bowel Syndrome. There is a need for educational strategies aimed at promoting health, including the adoption of healthier lifestyle habits such as physical activity, sleep hygiene and control of situations that induce awakening. Finally, more quality studies are needed to investigate risk factors for poor sleep quality in medical students.

Los estudiantes de medicina son una población susceptible de dormir mal debido al estrés, obligaciones académicas y otros factores. Así, el objetivo de este trabajo fue recopilar información de la literatura sobre el tema, ante la creciente relevancia del tema en el campo de la salud pública. Esta es una Revisión de Literatura Integrativa que utilizó descriptores en DeCS y las bases de datos SciELO, BVS, LILACS y MEDLINE. Solo se consideraron los estudios que utilizaron el índice PSQI. La búsqueda en las bases de datos resultó inicialmente en 140 artículos, luego de aplicar los criterios de exclusión e inclusión, 18 artículos resultaron aptos para la muestra final. La mayoría de los artículos (70%) mostraron una mala calidad del sueño entre los estudiantes, pero menos del 15% tenía un trastorno del sueño. La media de los índices PSQI disponibles fue 6,24 y el factor más importante asociado con la mala calidad del sueño fueron: depresión, ansiedad, estrés y puntuaciones más altas para los estudiantes con síndrome de intestino irritable. Existe necesidad de estrategias educativas orientadas a promover la salud, incluyendo la adopción de hábitos de vida más saludables como actividad física, higiene del sueño y control de situaciones que inducen el despertar. Finalmente, se necesitan más estudios de calidad para investigar los factores de riesgo de la mala calidad del sueño en estudiantes de medicina.O sono é uma função biológica essencial do ser humano relacionado à qualidade de vida, que pode ser avaliado por meio de escalas como o “Índice de Qualidade do Sono de Pittsburgh”. Estudantes de medicina são uma população suscetível a ter sono insatisfatório por estresse, obrigações acadêmicas e outros fatores. Assim, o objetivo desse trabalho foi unir informações da literatura a respeito da temática, tendo em vista a crescente relevância do assunto no campo da saúde pública. Trata-se de uma Revisão Integrativa da Literatura (RIL) que utilizou descritores no DeCS e as bases SciELO, BVS, LILACS e MEDLINE. Foram considerados apenas estudos que usavam o índice PSQI.  A busca nas bases resultou inicialmente em 140 artigos, após a aplicação dos critérios de exclusão e inclusão, 18 artigos foram aptos para a amostra final. A maioria dos artigos (70%>) apresentaram má qualidade de sono entre os estudantes, mas menos de 15% possuiu distúrbio de sono. A média dos índices PSQI disponibilizados foi de 6,24 e o maior fator de associação da má qualidade de sono foram: depressão, ansiedade, estresse e pontuações maiores para estudantes com Síndrome do Intestino Irritável. Há a necessidade de estratégias educacionais voltadas à promoção da saúde, incluindo a adoção de hábitos de vida mais saudáveis como atividade física, higiene do sono e controle de situações que induzam o despertar.
