Analysis of the treatment of supracondylar fractures of the pediatric humerus in a referral hospital

Resumo: Objective: To evaluate the treatment profile of supracondylar fractures of the pediatric humerus in a referral hospital in northeastern Brazil. Methodology: This is a retrospective observational study of data on patients who were treated at a pediatric urgent and emergency care referral unit of the Brazilian National Health System (SUS) between July 2022 and June 2023. Patients under the age of 13 diagnosed with a supracondylar humerus fracture were included in the study. Results: 197 patients were assessed, with a mean age of 5.4 years. Of these, 88 patients underwent surgery (44.6%), with a mean age of 5.3 years at the time of surgery and a predilection for males (54.5%). Of the patients who underwent surgery, 10 (11.5%) had to undergo open reduction to properly position the fracture. The main fixation pattern in the sample was fixation with crossed Kirschner wires (65.9%) and the majority of fixations were carried out using 2 Kirschner wires (76.5%). There was a statistically significant association between fracture severity (Gartland types III and IV) and open reduction. No type II cases had complications, while 3 type III and 2 type IV cases had complications. Conclusion: The data obtained is in line with the worldwide literature, except for the lower frequency of the general surgical approach and the higher frequency of cross-fixation in this study.

Metodología: Se trata de un estudio observacional retrospectivo de datos de pacientes que fueron tratados en una unidad de referencia de urgencias y emergencias pediátricas del Sistema Único de Salud (SUS) entre julio de 2022 y junio de 2023. Fueron incluidos en el estudio pacientes menores de 13 años diagnosticados de fractura supracondílea de húmero. Resultados: Se evaluaron 197 pacientes, con una edad media de 5,4 años. De ellos, 88 pacientes fueron intervenidos (44,6%), con una edad media de 5,3 años en el momento de la cirugía y una predilección por los varones (54,5%). De los pacientes intervenidos, 10 (11,5%) tuvieron que someterse a reducción abierta para posicionar adecuadamente la fractura. El patrón de fijación principal en la muestra fue la fijación con agujas de Kirschner cruzadas (65,9%) y la mayoría de las fijaciones se realizaron utilizando 2 agujas de Kirschner (76,5%). Hubo una asociación estadísticamente significativa entre la gravedad de la fractura (tipos III y IV de Gartland) y la reducción abierta. Ningún caso de tipo II presentó complicaciones, mientras que 3 casos de tipo III y 2 de tipo IV tuvieron complicaciones. Conclusión: Los datos obtenidos concuerdan con la literatura mundial, excepto por la menor frecuencia del abordaje quirúrgico general y la mayor frecuencia de la fijación cruzada en este estudio.Objetivo: Avaliar o perfil de tratamento das fraturas supracondilianas do úmero pediátrico em um hospital de referência do Nordeste brasileiro. Metodologia: trata-se de um estudo observacional retrospectivo dos dados dos pacientes que foram atendidos em uma unidade de referência de urgência e emergência pediátrica do Sistema Único de Saúde (SUS), no período de julho de 2022 a junho de 2023. Foram incluídos no estudo os pacientes menores de 13 anos que receberam o diagnóstico de fratura supracondiliana do úmero. Resultados: foram avaliados 197 pacientes com idade média de 5,4 anos. Desses, 88 pacientes foram submetidos a procedimento cirúrgico (44,6%), com idade média de 5,3 anos no momento da cirurgia e predileção pelo sexo masculino (54,5%). Dos pacientes operados, 10 (11,5%) tiveram que ser submetidos a redução aberta para adequado posicionamento da fratura. O principal padrão de fixação da amostra foi a fixação com fios de Kirschner cruzados (65,9%) e a maioria das fixações foi realizada utilizando-se 2 fios de Kirschner (76,5%). Observou-se uma associação estatisticamente significativa entre a gravidade da fratura (tipo III e IV de Gartland) e a realização de redução aberta. Nenhum caso operado do tipo II apresentou complicações, enquanto 3 casos do tipo III e 2 casos do tipo IV apresentaram complicações.
