Encefalopatia hepática: uma revisão narrativa

Resumo: The liver is an indispensable organ for maintaining our body's homeostasis, being responsible for important functions such as the elimination of toxins. It has a considerable capacity for regeneration, however, when these aggressions do not cease, regeneration occurs incorrectly, resulting in scars and nodules. Given the epidemiological importance of HE, this article aims to discuss the pathophysiological mechanisms of the disease. This study is a narrative review with the purpose of discussing and describing HS and its pathophysiology, as well as demonstrating how this syndrome affects the personal lives of affected individuals. The following databases were used: SciElo (Scientific Electronic Library Online) and PubMed (US National Library of Medicine), with scientific data from January 2, 2023, with no language restriction and no restriction on the year of publication. Several studies show that the major neuroinhibitory system, GABA, is increased in HE. Furthermore, an important consideration for ammonia in the face of the syndrome must be exposed; the receptors that are located on the periphery of the mitochondria, due to the presence of ammonia, as well as ligandins, triggering the activation of neurosteroid production in astrocytes. In any case, all studies have stated that the later the diagnosis is made, the more severe the systemic consequences of the pathology will be. In addition, it is of fundamental importance that the medical centers responsible for offering hospital support to patients affected by HE are prepared.

Tiene una considerable capacidad de regeneración, sin embargo, cuando estas agresiones no cesan, la regeneración se produce de forma incorrecta, dando lugar a cicatrices y nódulos. Dada la importancia epidemiológica de la EH, este artículo tiene como objetivo discutir los mecanismos fisiopatológicos de la enfermedad. Este estudio es una revisión narrativa con el objetivo de discutir y describir la HS y su fisiopatología, así como demostrar cómo este síndrome afecta la vida personal de los individuos afectados. Se utilizaron las siguientes bases de datos: SciElo (Scientific Electronic Library Online) y PubMed (US National Library of Medicine), con datos científicos desde el 2 de enero de 2023, sin restricción de idioma y sin restricción de año de publicación. Varios estudios muestran que el principal sistema neuroinhibidor, GABA, está aumentado en la EH. Además, se debe exponer una consideración importante para el amoníaco ante el síndrome; los receptores que se ubican en la periferia de la mitocondria, debido a la presencia de amoníaco, así como de ligandinas, desencadenan la activación de la producción de neuroesteroides en los astrocitos. En cualquier caso, todos los estudios han afirmado que cuanto más tardío sea el diagnóstico, más graves serán las consecuencias sistémicas de la patología. Además, es de fundamental importancia que los centros médicos encargados de brindar apoyo hospitalario a los pacientes afectados por EH estén preparados.O fígado é um órgão indispensável para a manutenção da homeostase do nosso corpo, sendo responsável por funções importantes como a eliminação de toxinas. Diante da importância epidemiológica do EH, este artigo visa discutir os mecanismos fisiopatológicos da doença. Este estudo trata-se de uma revisão narrativa com o propósito de discutir e descrever sobre a EH e sua fisiopatologia, como também, demonstrar como esta síndrome afeta a vida pessoal dos indivíduos acometidos. Foi utilizado o banco de dados: SciElo (Scientific Eletronic Library Online) e PubMed (US National Library of Medicine), com dados científicos de 2 de janeiro de 2023, sem restrição de idioma e sem restrição ao ano de publicação. Vários estudos mostram que o maior sistema neuroinibitório, GABA, está aumentado na EH. Ademais, uma importante consideração pela amônia diante da síndrome deve ser exposta; os receptores que ficam localizados na periferia da mitocôndria, pela presença da amônia, assim como de ligandinas, provocando a ativação da produção de neuroesteroides nos astrócitos. De qualquer maneira, todas as pesquisas afirmaram que quanto mais tardiamente é feito o diagnóstico, mais grave serão as consequências sistêmicas da patologia.
