Resumo: The abuse of drugs, legal or illegal, is one of the main social and public health problems in the world, so the contact of the dental surgeon with patients dependent on these substances is inevitable. Studies suggest that drug use increases oral manifestations, such as changes in salivary flow, dental wear, caries, periodontal disease, dry mouth, and pain. Therefore, the professional must be prepared to assist these patients and be aware of the limitations and possible interactions, especially in invasive treatments, whether in a hospital environment with the sedated patient or in the office with local anesthetics. The objective of this work is to report a case of monitoring blood pressure, oxygen saturation and heart rate, during a procedure with multiple extractions in a chemical dependent patient. Although the patient remained stable, there was an increase in diastolic pressure in the postoperative period, which may be related to the vasoconstriction caused by the use of marijuana and crack, and the action of the anesthetic's epinephrine. This increase is related to changes in the sympathetic and parasympathetic system, which is why the importance of knowing the substances used in the procedure and in prescription drugs. It is concluded that it is necessary to improve the knowledge of the physiological, chemical and neurological changes that involve these patients, for a safe dental treatment, and when necessary in conjunction with a multi-professional team, to know how to refer the patient and identify health problems that are not only oral.

Los estudios sugieren que el uso de drogas aumenta las manifestaciones orales, como cambios en el flujo salival, desgaste dental, caries, enfermedad periodontal, boca seca y dolor. Por lo tanto, el profesional debe estar preparado para ayudar a estos pacientes y conocer las limitaciones y las posibles interacciones, especialmente en tratamientos invasivos, ya sea en un entorno hospitalario con el paciente sedado o en el consultorio con anestésicos locales. El objetivo de este trabajo es informar un caso de monitoreo de la presión arterial, la saturación de oxígeno y la frecuencia cardíaca, durante un procedimiento con múltiples extracciones en un paciente químico dependiente. Aunque el paciente permaneció estable, hubo un aumento de la presión diastólica en el postoperatorio, que puede estar relacionado con la vasoconstricción causada por el uso de marihuana y crack, y la acción de la epinefrina del anestésico. Este aumento está relacionado con cambios en el sistema simpático y parasimpático, razón por la cual es importante conocer las sustancias utilizadas en el procedimiento y en los medicamentos recetados. Se concluye que es necesario mejorar el conocimiento de los cambios fisiológicos, químicos y neurológicos que involucran a estos pacientes, para un tratamiento dental seguro y, cuando sea necesario, junto con un equipo multiprofesional, para saber cómo derivar al paciente e identificar problemas de salud que no sean solo orales.O abuso de drogas, lícitas ou ilícitas, é um dos principais problemas sociais e de saúde pública do mundo, sendo assim o contato do cirurgião dentista com pacientes dependentes dessas substâncias é inevitável. Estudos sugerem que o uso de drogas aumenta as manifestações orais, como alterações do fluxo salivar, desgaste dental, cárie, doença periodontal, xerostomia e dor. Por isso, o profissional deve estar preparado para atender esses pacientes, e ter conhecimento sobre as limitações e possíveis interações, principalmente em tratamentos invasivos, seja em ambiente hospitalar com o paciente sedado ou no consultório com anestésicos locais. O objetivo deste trabalho é relatar um caso de monitoramento da pressão arterial, saturação de oxigênio e frequência cardíaca, durante um procedimento com múltiplas exodontias em uma paciente dependente química. Apesar da paciente se manter estável, foi constatado aumento da pressão diastólica no pós-operatório, podendo estar relacionado com a vasoconstrição causada pelo uso da maconha e crack, e a ação da epinefrina do anestésico. Esse aumento se relaciona com alterações no sistema simpático e parassimpático, por isso a importância de se conhecer as substâncias empregadas no procedimento e em medicamentos receitados.
