The importance of hypertensive control in chronic renal patients: a literature review

Resumo: Systemic arterial hypertension (SAH) is characterized by increased blood pressure, influenced by several factors (heredity, socioeconomic aspects and lifestyle) and affecting several target organs, such as the heart, vessels, brain and kidneys. Given this, among the main diseases caused by SAH, chronic kidney disease (CKD) stands out. This pathology is defined as the progressive and irreversible loss of nephrons (functional units of the kidney), with a consequent decrease in the glomerular filtration rate and/or increased excretion of albumin in the urine. In this regard, the main objective of this scientific article is to discuss the importance of blood pressure control in the treatment of chronic kidney disease, addressing relevant topics on the subject and bringing a review of the current bibliography. Thus, 58 articles were selected, which went through exclusion criteria (identification, screening, eligibility and inclusion), totaling, for the final analysis, 22 articles. The search was carried out in the following databases: Virtual Health Library (BVS), Scientif Electronic Library Online (SciELO), National Library of Medicine (PubMed), and Google School. Furthermore, the descriptors used were: “chronic kidney disease”, “hypertension”, “diagnosis”, “treatment”, “pressure control”; and to cross words, the Boolean operator “and” was used. The searches were limited to articles published between 2018 and 2022, in Portuguese, English or Spanish. After the analysis, it was evident that hypertensive control, both through pharmacological interventions and through behavioral and lifestyle changes, brought beneficial results for chronic kidney disease patients, reducing the progression of CKD.

Ante esto, entre las principales enfermedades causadas por la HAS, se destaca la enfermedad renal crónica (ERC). Esta patología se define como la pérdida progresiva e irreversible de nefronas (unidades funcionales del riñón), con la consiguiente disminución de la tasa de filtración glomerular y/o aumento de la excreción de albúmina en la orina. En ese sentido, el objetivo principal de este artículo científico es discutir la importancia del control de la presión arterial en el tratamiento de la enfermedad renal crónica, abordando temas relevantes sobre el tema y trayendo una revisión de la bibliografía actual. Así, fueron seleccionados 58 artículos, que pasaron por criterios de exclusión (identificación, tamizaje, elegibilidad e inclusión), totalizando, para el análisis final, 22 artículos. La búsqueda se realizó en las siguientes bases de datos: Virtual Health Library (BVS), Scientif Electronic Library Online (SciELO), National Library of Medicine (PubMed) y Google School. Además, los descriptores utilizados fueron: “enfermedad renal crónica”, “hipertensión”, “diagnóstico”, “tratamiento”, “control de presión”; y para cruzar palabras se utilizó el operador booleano “y”. Las búsquedas se limitaron a artículos publicados entre 2018 y 2022, en portugués, inglés o español. Después del análisis, se evidenció que el control de la hipertensión, tanto a través de intervenciones farmacológicas como a través de cambios de comportamiento y estilo de vida, trajo resultados beneficiosos para los pacientes con enfermedad renal crónica, reduciendo la progresión de la ERC.A hipertensão arterial sistêmica (HAS) é caracterizada pelo aumento da pressão arterial, sofrendo influência de vários fatores (hereditariedade, aspectos socioeconômicos e hábitos de vida) e afetando diversos órgãos-alvo, como o coração, os vasos, o cérebro e os rins. Visto isso, dentre as principais enfermidades causadas pela HAS, destaca-se a doença renal crônica (DRC). Essa patologia é definida como a perda progressiva e irreversível dos néfrons (unidades funcionais do rim), com consequente diminuição da taxa de filtração glomerular e/ou aumento da excreção de albumina na urina. Nesse aspecto, o presente artigo científico tem como principal objetivo dissertar sobre a importância do controle pressórico no tratamento do doente renal crônico, abordando tópicos pertinentes sobre o tema e trazendo uma revisão da bibliografia atual. Dessa forma, foram selecionados 58 artigos, os quais passaram por critérios de exclusão (identificação, triagem, elegibilidade e inclusão), totalizando, para a análise final, 22 artigos. A busca foi feita nos seguintes bancos de dados: Biblioteca Virtual de Saúde (BVS), Scientif Eletronic Library Online (SciELO), National Library of Medicine (PubMed), e Google School. Ademais, os descritores utilizados foram: “doença renal crônica”, “hipertensão arterial”, “diagnóstico”, “tratamento”, “controle pressórico”; e para o cruzamento das palavras utilizou-se o operador booleano “and”. As buscas se limitaram em artigos publicados entre 2018 e 2022, em português, inglês ou espanhol.
