Oral lipectomy: mediate surgical complication report

Resumo: The buccal fat pad described as a fatty tissue located on the face and its removal is known as buccal lipectomy or bichectomy, and may be indicated as aesthetic and functional purpose. A 33-year-old female patient sought treatment because of bilateral morsicatio. Oral lipectomy surgery was performed and after three hours of the procedure, one patient reported disproportionate and unilateral facial edema, with presence of mild and late pain, having shown periorbital hematoma training and right eye bruising. One patient required further intervention to explore a wound requiring hematoma drainage, followed by pressure maneuvers and cauterization with a monopolar electric scalpel. In view of the stabilization of the bleeding, the patient was instructed to use antibiotics, as well as control pain with analgesics and cryotherapy, being monitored daily until the return of edema was evident. The hematoma was recorded at 30 days without functional and aesthetic impairment. Oral lipectomy surgery should be performed by a qualified professional with experience in an outpatient and hospital environment, with the ability to intervene early on possible complications, minimizing the possibility of progression to more significant complications.

Este trabajo tiene como objetivo reportar un caso de complicación mediada tras una lipectomía oral. Una paciente de 33 años buscó tratamiento por tener morsicatio bilateral. Se realizó cirugía de lipectomía oral y tres horas después de finalizado el procedimiento, la paciente refirió edema en la cara derecha desproporcionada y unilateral, con presencia de dolor leve y pulsátil, con evidente formación de hematoma periorbitario y hematoma en ojo derecho. El paciente requirió una nueva intervención para explorar la herida, requiriendo drenaje del hematoma, seguido de maniobras de compresión y cauterización con bisturí eléctrico monopolar. Ante la estabilización del sangrado, se instruyó a la paciente a utilizar antibióticos, así como a controlar el dolor con analgésicos y crioterapia, siendo monitorizada diariamente hasta que se evidencie la regresión del edema. El hematoma retrocedió a los 30 días, sin deterioro funcional ni estético. La cirugía de lipectomía oral debe ser realizada por un profesional cualificado y con experiencia en el ámbito ambulatorio y hospitalario, con capacidad para intervenir precozmente en las posibles complicaciones, minimizando la posibilidad de progresar a complicaciones más importantes.O coxim adiposo bucal é descrito como um tecido gorduroso localizado na face e a sua remoção é conhecida como lipectomia bucal ou bichectomia, podendo ser indicada com finalidade estética e funcional. Esse trabalho tem como objetivo relatar um caso de complicação mediata após a lipectomia bucal. Paciente, 33 anos, sexo feminino, buscou tratamento por apresentar morsicatio bilateral. Foi realizada cirurgia de lipectomia bucal e após três horas do fim do procedimento, a paciente relatou edema na face desproporcional e unilateral direito, com presença de dor leve e latejante, havendo evidente formação de hematoma periorbitário e equimose no olho direito. A paciente necessitou de nova intervenção para explorar a ferida, necessitando de drenagem do hematoma, seguida de manobras de compressão e cauterização com bisturi elétrico monopolar. Diante da estabilização do sangramento, a paciente foi orientada a fazer uso de antibiótico, bem como controlar a dor com analgésicos e crioterapia, sendo monitorada diariamente até que a regressão do edema fosse evidente. O hematoma regrediu com 30 dias, não havendo prejuízo funcional e estético.
