Anxiety and depression symptoms in patients with tinnitus: systematic review

Resumo: Objective: to compare tinnitus sufferers to non-sufferers, regarding the presence of symptoms of anxiety and depression. Methodology: PubMed and Lilacs databases were searched for cross-sectional, cohort or case-control studies, with adult humans. The studies have been selected through a two-step process (title/abstract screening and full text reading). The study selection, data extraction and assessment of study quality were made independently by two reviewers. Results: 29 studies were included, totalizing 1,208,435 participants. 78% and 76% of the studies found a significant (p < 0,05) positive association between tinnitus and depression and anxiety, respectively. All the studies that compared the non-annoying tinnitus subgroup to controls found a significant (p < 0,05) positive association between non-annoying tinnitus and depression. All the studies that assessed the correlation between tinnitus-related annoyance and anxiety or depression levels found a significant (p < 0,05) positive correlation. Conclusion: tinnitus sufferers have greater odds of anxiety and depression symptoms than non-sufferers, and such finding is independent of hearing loss. The more annoying the tinnitus, the greater the prevalence and severity of the negative emotional symptoms; however, even not annoying tinnitus is associated with greater odds of depression.

Metodología: se han realizado búsquedas en las bases de datos Pubmed y Lilacs, de estudios con seres humanos adultos, del tipo transversal, de casos y controles o de cohortes, que han sido seleccionados en dos etapas (lectura del título y del resumen y lectura de todo el texto). La selección, la extracción de los datos y la evaluación de la calidad de los estudios han sido realizadas por dos revisores independientes. Resultados: se han incluido 29 estudios, totalizando 1.208.435 sujetos. 78% e 76% de los estudios han encontrado asociación positiva y significativa (p < 0,05) entre acúfeno y síntomas depresivos y ansiosos, respectivamente. Todos los estudios que compararon los síntomas depresivos del subgrupo con acúfeno no molesto con los del grupo control han encontrado asociación positiva y significativa (p < 0,05). Todos los estudios que evaluaron la correlación entre el nivel de molestia del acúfeno y los niveles de ansiedad o depresión han encontrado correlación positiva y significativa (p < 0,05). Conclusión: la presencia de síntomas depresivos y ansiosos es mayor en pacientes con acúfeno do que la en controles, y esta asociación es independiente de pérdida auditiva. Cuanto más molesto sea el acúfeno, mayores serán la prevalencia y la intensidad de los síntomas psiquiátricos. Sin embargo, acúfenos están asociados a mayor probabilidad de presentar síntomas depresivos aun cuando sean acúfenos no molestos.Objetivo: comparar portadores e não-portadores de zumbido quanto à presença de sintomas depressivos e ansiosos. Metodologia: foram realizadas buscas nas bases de dados PubMed e Lilacs de estudos com humanos adultos, do tipo transversal, coorte ou caso-controle, os quais foram selecionados em duas etapas (leitura do título e resumo e leitura do texto inteiro). A seleção, extração dos dados e avaliação da qualidade dos artigos foram realizadas por dois revisores independentes. Resultados: foram incluídos 29 artigos, abrangendo 1.208.435 participantes. 78% e 76% dos artigos encontraram associação positiva e significativa (p < 0,05) do zumbido com depressão e ansiedade, respectivamente. Todos os artigos que compararam o subgrupo de portadores de zumbido não-incômodo com os controles quanto à depressão encontraram associação positiva e significativa (p < 0,05). Todos os artigos que avaliaram a correlação do incômodo do zumbido com os níveis de ansiedade ou depressão encontraram correlação positiva e significativa (p < 0,05). Conclusão: portadores de zumbido tem maior chance de apresentar sintomas depressivos e ansiosos que não portadores, e tal associação é independente da perda auditiva.
