Error analysis in prescriptions dispensed in a Basic Health Unit

Resumo: The study aims to evaluate the conformity of the prescriptions dispensed in a Basic Health Unit, according to RDC nº 357/2001. This was a cross-sectional, observational and retrospective study carried out from January to December 2017 at the Pharmacy of the Basic Health Unit of Vila Coelho Dias in Bacabal - Maranhão, using as sample the second copies of medical prescriptions that were retained in the pharmacy. During the analyzed period, 922 prescriptions were found; of these, 488 (52.9%) were excluded because they were considered transcriptions made by nurses. Of the total medical prescriptions analyzed, 42 (10%) were illegible, 96 (22%) legible and 296 (68%) poorly legible. The study also shows that 100% of the prescriptions have the patient's name, but age and address are absent in all prescriptions. Regarding the variables related to the prescription, 100% presented the date and signature of the prescriber and only 18 (4.15%) presented erasures. Note that 67.1% (n=291) reported dosage, concentration was absent in 34.4% (n=144), pharmaceutical form was present in 67.2% (n=292), duration of treatment was absent in 85.71% (n=372) and 49.1% (n=231) had the route of administration. It was found in the present study that prescription errors ranged from readability to the variables analyzed, both for the patient, prescriber and the drug, which allowed us to conclude that the legal aspects of the prescription continue to be violated by prescribers.

Se trata de un estudio transversal, observacional y retrospectivo realizado de enero a diciembre de 2017 en la Farmacia de la Unidad Básica de Salud de Vila Coelho Dias en Bacabal - Maranhão, utilizando como muestra las segundas copias de las recetas médicas que se conservaban en la farmacia. Durante el período analizado se encontraron 922 recetas, de las cuales se excluyeron 488 (52,9%) por considerarse transcripciones realizadas por enfermeras. Del total de prescripciones médicas analizadas, 42 (10%) eran ilegibles, 96 (22%) legibles y 296 (68%) poco legibles. El estudio también muestra que el 100% de las recetas tienen el nombre del paciente, pero la edad y la dirección están ausentes en todas las recetas. En cuanto a las variables relacionadas con la prescripción, el 100% presentó la fecha y firma del prescriptor y sólo 18 (4.15%) presentaron borraduras. Nótese que el 67,1% (n=291) informó la dosis, la concentración estaba ausente en el 34,4% (n=144), la forma farmacéutica estaba presente en el 67,2% (n=292), la duración del tratamiento estaba ausente en el 85,71% (n=372) y el 49,1% (n=231) tenía la vía de administración. En el presente estudio se encontró que los errores de prescripción iban desde la legibilidad hasta las variables analizadas, tanto para el paciente como para el prescriptor y el fármaco, lo que nos permitió concluir que los aspectos legales de la prescripción siguen siendo violados por los prescriptores.O estudo objetiva avaliar a conformidade das receitas dispensadas em uma Unidade Básica de Saúde, de acordo com a RDC nº 357/2001. Trata-se de um estudo exploratório, descritivo e quantitativo, realizado durante os meses de janeiro a dezembro de 2017 na Farmácia da Unidade Básica de Saúde da Vila Coelho Dias em Bacabal - Maranhão, utilizando como amostra as segundas vias das prescrições médicas que foram retidas na farmácia. Foram encontradas, durante o período analisado, 922 prescrições, dessas, 488 (52,9%) foram excluídas por serem consideradas transcrições realizadas por enfermeiros. Do total de prescrições médicas analisadas 42 (10%) encontraram-se ilegíveis, 96 (22%) legíveis e 296 (68%) pouco legíveis. Este estudo mostra ainda que 100% das prescrições apresentam o nome do paciente, porém, idade e endereço encontram-se ausentes em todas as prescrições. Quanto as variáveis relacionadas à prescrição, 100% apresentaram a data e assinatura do prescritor e apenas 18 (4,15%) apresentaram rasuras. Nota-se que 67,1% (n=291) informaram a posologia, a concentração estava ausente em 34,4% (n=144), forma farmacêutica estava presente em 67,2% (n=292), a duração do tratamento estava ausente em 85,71% (n=372) e 49,1% (n=231) continham a via de administração.
