Complications presented by patients with acute kidney disease that used furosemide associated or not albumin replacement

Resumo: Acute kidney injury or acute kidney injury (AKI) is a syndrome characterized by a sudden decline in kidney function. Furosemide is the most widely used diuretic in these conditions and studies have shown that both this drug and albumin are involved in inducing diuresis and natriuresis, thus decreasing complications in the short and long term. The aim of the present study is to evaluate the complications presented by patients with AKI who used furosemide associated or not with albumin replacement in an ICU. The medical records of patients admitted to the Hospital Menandro de Farias between the years 2012 to 2017 were analyzed. Patients admitted to the ICU, regardless of the diagnosis of entry, who had taken AKI and used furosemide associated or not with albumin, were included in the study. , and who needed dialysis or not. Patients known to have chronic kidney injury were excluded from the study. Patients who used only furosemide were less submitted to hemodialysis and had higher values of urea and serum creatinine at the end of hospitalization. There was no change in the hospital outcome among patients who used furosemide alone or associated with albumin. However, individuals who used only furosemide had a lower need to undergo hemodialysis therapy.

La furosemida es el diurético más utilizado en estas afecciones y los estudios han demostrado que tanto este fármaco como la albúmina están involucrados en la inducción de diuresis y natriuresis, disminuyendo así las complicaciones a corto y largo plazo. El objetivo del presente estudio es evaluar las complicaciones que presentan los pacientes con IRA que utilizaron furosemida asociada o no a la reposición de albúmina en una UCI. Se analizaron las historias clínicas de los pacientes ingresados en el Hospital Menandro de Farias entre los años 2012 a 2017. Se incluyeron en el estudio los pacientes ingresados en UCI, independientemente del diagnóstico de ingreso, que tuvieran LRA y usaran furosemida asociada o no a albúmina y quién necesitaba diálisis o no. Se excluyó del estudio a los pacientes que se sabía que tenían una lesión renal crónica. Los pacientes que utilizaron solo furosemida fueron menos sometidos a hemodiálisis y tuvieron valores más altos de urea y creatinina sérica al final de la hospitalización. No hubo cambios en el resultado hospitalario entre los pacientes que usaron furosemida sola o asociada con albúmina. Sin embargo, las personas que usaban solo furosemida tenían una menor necesidad de someterse a terapia de hemodiálisis.A injúria renal aguda ou lesão renal aguda (LRA) é uma síndrome caracterizada por um declínio súbito da função renal. A furosemida é o diurético mais utilizado nessas condições e estudos têm comprovado que tanto este fármaco como a albumina estão envolvidos na indução da diurese e natriurese, diminuindo assim, as complicações a curto e longo prazo. O objetivo do presente estudo é avaliar as complicações apresentadas por pacientes acometidos com LRA que fizeram uso de furosemida associada ou não a reposição de albumina em uma UTI. Foram analisados os prontuários de pacientes internados no Hospital Menandro de Farias entre os anos de 2012 a 2017. Sendo incluídos no estudo os pacientes internados na UTI, independente do diagnóstico de entrada, que cursaram com LRA e fizeram uso de furosemida associada ou não à albumina, e que necessitaram ou não de diálise. Os pacientes sabidamente portadores de lesão renal crônica foram excluídos do estudo. Os pacientes que fizeram uso apenas da furosemida foram menos submetidos a hemodiálise e apresentaram maiores valores de ureia e creatinina sérica ao fim do internamento. Não houve alteração no desfecho hospitalar entre os pacientes que fizeram uso da furosemida isolada ou associada a albumina.
