Are there differences in the characteristics of individuals with COPD who exacerbate or not during a pulmonary rehabilitation program?

Resumo: Objectives:To identify the characteristics of individuals with Chronic Obstructive Pulmonary Disease (COPD) that exacerbate or not during a exercise training (ET) program, and check if any these groups are less likely to the benefits of ET. Methods:Individuals with COPD were evaluated for lung function, respiratory muscle strength, body composition, 6-minute walk test (6MWT), peripheral muscle strength, quality of life, functional status, dyspnea sensation in daily life (Medical Research Council [MRC] scale), anxiety and depression. All individuals performed a high-intensity exercise training (ET) program with aerobic exercises and strength training (three times/week, 12 weeks). For analysis, the individuals were separated into two groups (Group that Exacerbated [GE] and Group that did Not Exacerbate [GNE], and compared as to their characteristics. Results: 44 individuals completed the ET (26 in the GNE [FEV1 46±16%predicted] and 18 in the GE [FEV1 42±18 %predicted]). In pre-training, GE individuals were younger, more anxious and with worse forced vital capacity than GNE. After the ET, only the GE showed significant improvement in dyspnea (pre vs post-training: MRC 4 [3–4] vs 3 [2–4]; P=0.03) and only the GNE in the 6MWT (478 [424-519] vs 516 [467–558]; P<0.0001). Both groups showed improvement significant in the peripheral muscle strength. Finally, considering the comparison of the change post-pre ET, the GNE had a more marked improvement in quality of life and 6MWT. Conclusion: Individuals who exacerbate during an ET are less likely to benefit from ET, and training strategies tailored t are necessary.

Métodos: Individuos con EPOC fueron evaluados para función pulmonar, fuerza muscular respiratoria, composición corporal, prueba de caminata de 6 minutos (6MWT), fuerza muscular periférica, calidad de vida, estado funcional, sensación de disnea em la vida diaria (escala Medical Research Council [MRC]), ansiedad y depresión. Individuos realizaron un programa de EF de alta intensidad con ejercicios aeróbicos y de fuerza (tres veces/semana, 12 semanas). Para el análisis, los individuos foran separados en dos grupos (Grupo que Exacerbó [GE] y Grupo que no exacerbó [GNE] y comparados em cuanto a sus características. Resultados: 44 individuos completaron el ET (26 en el GNE [VEF1 46±16%predicho] y 18 en el GE [VEF1 42±18%predicho]). En el preentrenamiento, los sujetos del GE eran más jóvenes, más ansiosos y con peor capacidad vital forzada. Después del ET, solo el GE mostró una mejora significativa en la disnea pre vs post entrenamiento: MRC (4[3–4] vs 3[2–4]; P=0,03) y solo el GNE en el 6MWT (478[424-519] vs 516[467–558]; P<0,0001). Finalmente, considerando la comparación del cambio post-pre ET, el GNE tuvo mejoría más pronunciada que el GE em calidad de vida y 6MWT. Conclusión: Los sujetos que exacerban durante el ET son menos susceptibles a los beneficios de el ET y necesitan sus propias estrategias de entrenamiento.Objetivos: Identificar características dos indivíduos com doença pulmonar obstrutiva crônica (DPOC) que exacerbam ou não durante programa de treinamento físico (TF), e verificar se algum destes grupos é menos suscetível aos benefícios do TF. Métodos: Indivíduos com DPOC foram avaliados quanto à função pulmonar, força muscular respiratória, composição corporal, teste da caminhada de 6 minutos (TC6min), força muscular periférica, qualidade de vida, estado funcional, sensação de dispneia na vida diária (escala Medical Research Council [MRC]), ansiedade e depressão. Os indivíduos realizaram programa de TF de alta intensidade com exercícios aeróbicos e de força (três vezes/semana, 12 semanas). Para análise os indivíduos foram separados em dois grupos (Grupo que Exacerbou [GE] e Grupo que Não Exacerbou [GNE]) e comparados quanto às suas características. Resultados: 44 indivíduos completaram o TF (26 no GNE [VEF1 46±16%predito] e 18 no GE [VEF1 42±18%predito]). No pré-treinamento, indivíduos do GE eram mais novos, mais ansiosos e com pior capacidade vital forçada do que o GNE. Após TF, apenas o GE apresentou melhora significativa na dispneia (pré vs pós-treinamento: MRC 4 [3–4] vs 3 [2–4]; P=0,03) e apenas o GNE no TC6min (478 [424-519] vs 516 [467–558]; P<0,0001). Ambos os grupos apresentaram melhora significativa da força muscular periférica. Por fim, considerando a comparação da mudança pós-pré TF, o GNE teve melhora mais acentuada do que o GE na qualidade de vida e TC6min.
