Imaging diagnosis: level of knowledge and rational use among Primary Care physicians and resident physicians

Resumo: Given the technological-scientific advances, the patient's demand for more exams and greater availability and functionality of imaging methods, in addition to the aging and clinical complexity of the population, there has been and continues to be significant growth in diagnostic imaging exams and services in last decades. There is a lack of standardization of radiology teaching in institutions, pointing to the need to reassess medical curricula to ensure the rational, conscious and effective use of imaging resources, with benefits for the physician, the patient and the health system. Thus, we sought to assess the level of knowledge of primary care physicians and internal medicine residents about imaging tests as well as their rational use through the correct indication of the methods in clinical situations of daily practice. The study was carried out with 123 physicians, members of primary health care and/or residents of a medical clinic in São Luis, MA, through a structured questionnaire containing questions about general knowledge and an indication of imaging methods. In general, there was a higher level of knowledge expressed by the mean percentage of correct answers (%m.a) among the group of resident physicians (%m.a= 85.7 ±6.4) when compared to AB doctors (%m.a=78.2 ±15), with less time since graduation (%m.a=83.35±11.1). Physicians working in emergency services (84.70±7%) had higher means. Although AB physicians have had good results in recommending imaging tests in specific situations, in most cases the residents.

Existe una falta de estandarización de la enseñanza de la radiología en las instituciones, lo que apunta a la necesidad de reevaluar los planes de estudios médicos para garantizar el uso racional, consciente y eficaz de los recursos de imágenes, con beneficios para el médico, el paciente y el sistema de salud. Así, se buscó evaluar el nivel de conocimiento de los médicos de atención primaria y residentes de medicina interna sobre las pruebas de imagen, así como su uso racional a través de la correcta indicación de los métodos en situaciones clínicas de la práctica diaria. El estudio fue realizado con 123 médicos, miembros de la atención primaria de salud y/o residentes de una clínica médica en São Luis, MA, a través de un cuestionario estructurado que contenía preguntas sobre conocimientos generales e indicación de métodos de imagen. En general, hubo mayor nivel de conocimiento expresado por porcentaje medio de aciertos (%m.a) entre el grupo de médicos residentes (%m.a= 85,7 ±6,4) en comparación con los médicos AB (%m.a=78,2 ±15), con menos tiempo desde la graduación (%m.a=83.35±11.1). Los médicos que trabajan en los servicios de emergencia (84,70±7%) tuvieron promedios más altos. Aunque los médicos de AB han tenido buenos resultados al recomendar pruebas de imagen en situaciones específicas, en la mayoría de los casos los residentes.Diante dos avanços tecnológico-científicos, da demanda do paciente por mais exames e maior disponibilidade e funcionalidade dos métodos de imagem, além do envelhecimento e complexidade clínica da população, houve e continua havendo um significativo crescimento dos exames de diagnóstico por imagem e dos serviços nas últimas décadas. Existe uma falta de padronização do ensino de radiologia nas instituições, apontando a necessidade de reavaliação dos currículos médicos para se garantir o uso racional, consciente e eficaz dos recursos de imagem, com benefícios para o médico, para o paciente e para o sistema de saúde. Assim, buscou-se avaliar o nível de conhecimento dos médicos da atenção primária e residentes de clínica médica sobre os exames de imagem bem como seu uso racional através da indicação correta dos métodos em situações clínicas da prática diária. O estudo foi realizado com 123 médicos, integrantes da atenção primária à saúde e/ou residentes de clínica médica e São Luis, MA, por meio de questionário estruturado contendo questões sobre conhecimentos gerais e indicação dos métodos de imagem. De forma geral, houve maior nível de conhecimento expressado por percentual médio de acerto (%m.a) entre o grupo de médicos residentes (%m.a= 85,7 ±6,4) quando comparados aos médicos da AB (%m.a=78,2±15), com menos tempo de formados (%m.a=83,35±11,1). Médicos que atuam em serviço de urgência (84,70±7%) apresentaram maiores médias.
