Treatment of chronic low back pain in obese patients

Summary: Obesity is a pathological state resulting from excess body fat, this disease occurs when there is an imbalance between energy intake and energy expenditure. Excess adipose tissue causes several abnormal biochemical reactions in the body, making it an inflamed environment and a risk factor for many other pathological conditions, especially in the musculoskeletal system. The lumbar spine, as it performs the weight bearing function of the upper body, ends up being the region most affected by obesity, bringing, among many consequences, chronic pain and functional disability for the patient. Thus, this article aims to elucidate the treatment of chronic low back pain caused by the pathophysiological processes of obesity. For this, a literature review of a basic nature was developed, with a bibliographic and descriptive approach. Articles were selected according to publication date, keywords and cost. In this way, 94 articles were found, of this amount, according to the inclusion and exclusion criteria, for the final version, 17 bibliographical references were used to compose the discussion. In the discussion, the treatment of chronic low back pain is approached, first presenting the various modalities of conservative, non-pharmacological and associated pharmacological treatment, followed by invasive treatment and, finally, surgical approaches for chronic low back pain. It is concluded, then, from this article, that there is a broad therapeutic arsenal for the treatment of chronic low back pain, making it possible to choose the patient's therapy according to the severity of the condition, the capacity of the professional who is assisting and the costs involved.

El exceso de tejido adiposo provoca varias reacciones bioquímicas anormales en el cuerpo, convirtiéndolo en un ambiente inflamado y un factor de riesgo para muchos otros trastornos patológicos, especialmente en el sistema musculoesquelético. La columna lumbar, al realizar la función de soporte de peso de la parte superior del cuerpo, termina siendo la región más afectada por la obesidad, trayendo, entre varias consecuencias, dolor crónico e incapacidad funcional para el paciente. Por tanto, este artículo tiene como objetivo dilucidar el tratamiento de la lumbalgia crónica provocada por los procesos fisiopatológicos de la obesidad. Para ello se desarrolló una revisión bibliográfica de carácter básico, con un enfoque bibliográfico y descriptivo. Los artículos fueron seleccionados según fecha de publicación, palabras clave y costo. De esta manera se encontraron 94 artículos, de esta cantidad, según los criterios de inclusión y exclusión, para la versión final se utilizaron 16 referencias bibliográficas para componer la discusión. En la discusión se aborda el tratamiento de la lumbalgia crónica, presentando primero las diversas modalidades de tratamiento conservador, no farmacológico y farmacológico asociado seguido del tratamiento invasivo y, finalmente, los abordajes quirúrgicos para la lumbalgia crónica. Se concluye, de este artículo, que existe un amplio arsenal terapéutico para el tratamiento de la lumbalgia crónica, permitiendo elegir la terapia del paciente según la gravedad del cuadro, la capacidad del profesional que lo atiende y los costos. involucrado.Obesidade é um estado patológico resultante do excesso de gordura corporal, tal enfermidade ocorre quando há um desequilíbrio entre a ingestão e o gasto energético do indivíduo. O excesso de tecido adiposo provoca diversas reações bioquímicas anormais no organismo tornando este um ambiente inflamado e fator de risco para muitos outros acometimentos patológicos, principalmente no sistema músculo esquelético. A coluna lombar, como desempenha a função de sustentação de peso da parte superior do corpo, acaba sendo a região mais acometida pela obesidade trazendo, dentre várias consequências, a dor crônica e incapacidade funcional do doente. Desta forma, o presente artigo tem como objetivo elucidar o tratamento da dor lombar crônica ocasionada pelos processos fisiopatológicos da obesidade. Para isso, desenvolveu-se uma revisão de literatura narrativa simples, com uma abordagem bibliográfica e descritiva. Os artigos foram selecionados conforme data de publicação, palavras-chave e custo. Desta forma, foram encontrados 94 artigos, desse montante, conforme os critérios de inclusão e exclusão, para a versão final foram utilizadas 17 referências bibliográficas para compor a discussão. Na discussão, o tratamento da lombalgia crônica é abordado apresentando, primeiro, as várias modalidades de tratamento conservador, não farmacológico e farmacológico associado seguido do tratamento invasivo e, por fim, as abordagens cirúrgicas para a dor lombar crônica.
