Bioabsorbable fixation system as an alternative for the treatment of mandibular fractures in pediatric patients: literature review

Resumo: Although facial fractures in pediatric patients are uncommon due to bone elasticity, the mandible has a high prevalence in pediatric trauma. Metal fixation systems are conventionally used to treat mandibular fractures. However, their applications have been questioned in this patient profile because they have disadvantages such as limitation in mandibular growth, potential to cause infections and possible need for a second surgical procedure for their removal. The present study aims to know the particularities of the pediatric mandible, understand the functioning of bioabsorbable plates and discuss the positive and negative points of their use in the treatment of mandibular fractures. This is a narrative literature review carried out through a vast search in books, monographs and scientific articles from the last 10 years, in the PubMed, MEDLINE and Google Scholar databases, in the "advanced search" mode, using the following descriptors in Portuguese and English: “Mandibular fractures”, “Internal fixation of fractures” and “Pediatrics”. The advent of bioabsorbable fixation systems denotes several benefits regarding their application in the treatment of mandibular fractures in pediatric patients, due to their properties that allow resorption in the fixation system as bone repair is established. It was possible to consider that the use of bioabsorbable fixation as a treatment for mandibular fractures in children is a viable alternative, presenting benefits for not compromising mandibular growth, reinforcing the importance of carrying out future research in order to obtain higher levels of scientific evidence.

Los sistemas de fijación de metal se utilizan convencionalmente para tratar las fracturas mandibulares. Sin embargo, sus aplicaciones han sido cuestionadas en este perfil de pacientes debido a que presentan desventajas como limitación en el crecimiento mandibular, potencial para causar infecciones y posible necesidad de un segundo procedimiento quirúrgico para su extirpación. El presente estudio tiene como objetivo conocer las particularidades de la mandíbula pediátrica, comprender el funcionamiento de las placas bioabsorbibles y discutir los puntos positivos y negativos de su uso en el tratamiento de las fracturas mandibulares. Se trata de una revisión narrativa de la literatura realizada a través de una amplia búsqueda en libros, monografías y artículos científicos de los últimos 10 años, en las bases de datos PubMed, MEDLINE y Google Scholar, en la modalidad de "búsqueda avanzada", utilizando los siguientes descriptores en portugués y Inglés: “Fracturas mandibulares”, “Fijación interna de fracturas” y “Pediatría”. El advenimiento de los sistemas de fijación bioabsorbibles denota varios beneficios en cuanto a su aplicación en el tratamiento de fracturas mandibulares en pacientes pediátricos, debido a sus propiedades que permiten la reabsorción en el sistema de fijación a medida que se establece la reparación ósea. Se pudo considerar que el uso de la fijación bioabsorbible como tratamiento de las fracturas mandibulares en niños es una alternativa viable, presentando beneficios para no comprometer el crecimiento mandibular, reforzando la importancia de realizar futuras investigaciones para obtener mayores niveles de evidencia científica.Apesar das fraturas faciais em pacientes pediátricos se mostrarem pouco comuns devido à elasticidade óssea, a mandíbula possui uma grande prevalência nos traumas pediátricos. Sistemas de fixação metálicos convencionalmente são usados como tratamento de fraturas mandibulares. Todavia, suas aplicações vêm sendo questionadas neste perfil de paciente por apresentarem desvantagens como limitação no crescimento mandibular, potencial de causar infecções e possível necessidade de um segundo procedimento cirúrgico para sua remoção. O presente estudo tem como objetivo conhecer as particularidades da mandíbula pediátrica, entender o funcionamento das placas bioabsorvíveis e debater os pontos positivos e negativos do seu uso no tratamento das fraturas mandibulares. Trata-se de uma revisão de literatura do tipo narrativa realizada por meio de uma vasta pesquisa em livros, monografias e artigos científicos dos últimos 10 anos, nas bases de dados PubMed, MEDLINE e Google Acadêmico, no modo “pesquisa avançada”, utilizando os seguintes descritores na língua portuguesa e inglesa: “Fraturas mandibulares”, “Fixação interna de fraturas” e “Pediatria”. O advento dos sistemas de fixação bioabsorvível denota vários benefícios quanto a sua aplicação no tratamento de fraturas mandibulares em pacientes pediátricos, devido as suas propriedades que permitem a reabsorção no sistema de fixação na medida em que o reparo ósseo for estabelecido.
