Bidirectional association between periodontitis and type 2 diabetes mellitus: a literature review

Resumo: The link that periodontitis has with systemic diseases such as type 2 diabetes mellitus (T2DM) has been studied for a long time. Current evidence suggests that the mechanisms underlying this link are related to the inflammatory mediators present in both diseases and indicates that both diseases represent a mutual risk factor for developing each disease, respectively. Likewise, the impact that periodontal treatment can have on T2DM has been extensively studied, which could develop new treatment guidelines for this metabolic disease. However, this situation is currently not clear despite the clinical studies that have been carried out. It is suggested that such treatment can improve glycemic control and complications related to diabetes. The objective of this present review is to inform about the possible pathophysiological mechanisms that link T2DM with periodontitis in a bidirectional way. In the same way, to approach in a general way the effect that periodontal treatment can have on control of T2DM.

La evidencia actual sugiere que los mecanismos que subyacen a dicho vínculo están relacionados con los mediadores inflamatorios presentes en ambas enfermedades, y señala que además ambas enfermedades representan un factor de riesgo de manera mutua para desarrollar cada enfermedad, respectivamente. Asimismo, se ha estudiado ampliamente el impacto que puede tener el tratamiento periodontal sobre la DM2, lo cual pude desarrollar nuevas pautas de tratamiento para esta enfermedad metabólica. Sin embargo, actualmente dicha situación no está muy clara pese a los estudios clínicos que se han realizado. Se sugiere que dicho tratamiento puede mejor el control glucémico y disminuir complicaciones relacionadas con la diabetes. El objetivo de la presente revisión es informar sobre los posibles mecanismos fisiopatológicos que relacionan de forma bidireccional a DM2 con la periodontitis. De igual manera, abordar de forma general el efecto que puede tener el tratamiento periodontal sobre el control de la DM2.A ligação que a periodontite tem com doenças sistémicas como a Diabetes Mellitus tipo 2 (DM2) tem sido estudada há muito tempo. Os dados atuais sugerem que os mecanismos subjacentes a esta ligação estão relacionados com os mediadores inflamatórios presentes em ambas as doenças, e apontam que ambas as doenças representam um fator de risco mútuo para o desenvolvimento de cada doença, respectivamente.  Do mesmo modo, o impacto que o tratamento periodontal pode ter no DM2 foi amplamente estudado, o que pode levar ao desenvolvimento de novas directrizes de tratamento para esta doença metabólica. No entanto, esta situação não é atualmente muito clara, apesar dos estudos clínicos que tem sido realizados. Sugere-se que tal tratamento pode melhorar o controlo glicémico e diminuir as complicações relacionadas com a diabetes. O objetivo da presente revisão é informar sobre os possíveis mecanismos fisiopatológicos que bi-direcionalmente relacionam o DM2 com a periodontite.
