Self-screening as a tool for assessing the nutritional risk in hospitalized adults

Resumo: The present study aimed to assess the applicability of a nutritional self-screening tool in adults. This is a cross-sectional study, with male and females, admitted to a public hospital in Pernambuco. Nutritional self-screening was performed by eligible patients and by the researcher. The differences between the self-reported and measured weight and height were assessed by Wilcoxon’s test, with subsequent construction of the Bland & Altman graph. The chi-square test measured whether clinical and sociodemographic variables were associated with nutritional risk diagnosed by the researcher. The study included 171 patients, of which 59.1% were females, and mean age 51.7 ± 15.3 years. A considerable percentage (47.9%, n=82) could not screen themselves, mainly because of low education level and and advanced stage of the disease. The mean self-reported weight (68.4 ± 15.0 Kg) was similar to the weight measured (68.2 ± 15.1 kg). According to the Kappa coefficient, the two diagnoses had moderate agreement (k=0.62; p=0.001). The agreements between self-reported and measured weight and height were good, with a bias close to zero. It is concluded that the applicability of the self-screening instrument was not satisfactory, due to the low level of education and lack of understanding by the population studied.

Estudio transversal, con pacientes de ambos sexos, internados en un hospital público de Pernambuco. Se trata de un autocribado nutricional, realizado por pacientes e investigador. Se midió el peso/talla y se comparó con los datos informados por los pacientes. Para evaluar la diferencia se utilizó la prueba de Wilcoxon y se construyó el gráfico de Bland & Altman. La asociación entre las variables clínicas y sociodemográficas, con el riesgo nutricional identificado por el evaluador, se analizó mediante la prueba de chi-cuadrado. Muestra de 171 pacientes, en su mayoría mujeres (59,1%), edad 51,7 ± 15,3 años. Se encontró que el 47,9% (n=82) no pudo realizar el autoexamen, motivado por el bajo nivel educativo y el estado avanzado de la enfermedad. El peso informado por el paciente (68,4Kg ± 15,0) fue similar al medido por el investigador (68,2Kg ± 15,1). Según Kappa, hubo concordancia moderada entre los dos diagnósticos (k=0,62; p=0,001). En el análisis de peso y talla hubo buena concordancia, con un sesgo cercano a cero en ambas valoraciones. Se concluye que la aplicabilidad del instrumento de autoevaluación no fue satisfactoria, debido al bajo nivel educativo y falta de comprensión por parte de la población estudiada.Objetivou-se avaliar a aplicabilidade de instrumento de autotriagem nutricional em adultos. Foram aferidos peso/altura e comparados com os dados referidos pelos pacientes. Na avaliação da diferença foi usado o teste de Wilcoxon, sendo construído o gráfico de Bland & Altman. A associação entre variáveis clínicas e sociodemográficas, com o risco nutricional identificado pelo avaliador, foi analisado pelo teste do Qui-quadrado. Amostra de 171 pacientes, maioria mulheres (59,1%), de 51,7 ± 15,3 anos. Detectou-se que  47,9% (n=82) não conseguiram realizar a autotriagem, motivados por baixo grau de instrução e fase avançada da doença. O peso referido pelo paciente (68,4Kg ± 15,0) foi semelhante ao aferido pelo pesquisador (68,2Kg ± 15,1). Pelo Kappa houve moderada concordância entre os dois diagnósticos (k=0,62; p=0,001). Na análise de peso e altura, verificou-se uma boa concordância, com viés próximo a zero nas duas avaliações.
