LITERATURE REVIEW ON MUCOSITIS AND A PROPOSED PROTOCOL FOR THE PREVENTION AND TREATMENT OF ORAL COMPLICATIONS AFTER AN ANTINEOPLASTIC TREATMENT

Resumo: | Introduction: Oral cancer is the sixth most common cancer in the world. It occurs in approximately 6% of all diagnosed cancer cases. Surgery, radiotherapy, and chemotherapy procedures are the forms used in the treatment of head and neck cancer. According to the radiation dose, the form of administration, the extent and location of the irradiated area, the quality and intensity of radiation, and the patient’s individual factors, radiotherapy may cause some side effects in the oral cavity and gastrointestinal tract. Among them are mucositis, xerostomia, osteoradionecrosis, trismus, oral infection, dysphagia, radiation cavities, and periodontal changes. All patients in this treatment develop some degree of oral mucositis, the severity being influenced by patient and treatment factors. Complications of head and neck cancer are among the most devastating in the short and long term, because they affect the most basic human activities, such as eating and speaking. Objective: The aim of this study was to conduct a literature review on oral mucositis and propose a protocol for its prevention and treatment. Material and methods: For this literature review, the study made use of Pub Med, Sciencdirect, Elsevier, Capes, Bireme, Scielo, Medline, Lilacs, and BBO databases. Results: Mucositis is clinically classified into five levels, ranging from erythematic to ulceration, with or without impairment of eating and swallowing activities. Multiple preventive and healing treatments are described in literature for systemic and local approach. Oral hygiene care before, during, and after radiotherapy or chemotherapy is essential to minimize the side effects. Conclusion: Dentists have an important role in the multidisciplinary team in order to prevent, diagnose, and treat patients undergoing cancer treatments. |

Introdução: O câncer bucal é o sexto tipo de câncer mais comum em todo o mundo, com uma participação aproximada de 6% dos casos de câncer diagnosticados. A cirurgia, radioterapia e quimioterapia são formas terapêuticas utilizadas no tratamento de câncer de cabeça e pescoço. A radioterapia pode, segundo a dose de radiação, a forma de administração, a extensão e a localização da área irradiada, bem como a qualidade e poder de penetração da radiação e dos fatores individuais do paciente, provocar alguns efeitos adversos na cavidade bucal e trato gastrointestinal. Entre eles estão a mucosite, xerostomia, osteorradionecrose, trismo, infecção bucal, disfagia, cárie de radiação e alterações periodontais. Todos os pacientes sob este tratamento desenvolvem algum grau de mucosite oral, sendo sua severidade influenciada por fatores do paciente e fatores do tratamento. As complicações do câncer de cabeça e pescoço encontram-se entre as mais devastadoras, no curto e no longo prazo, por afetarem as atividades humanas mais básicas, tais como a alimentação e a comunicação. Objetivo: O objetivo deste trabalho foi realizar uma revisão de literatura, principalmente sobre a mucosite, e propor um protocolo para sua prevenção e tratamento. Material e métodos: Bases de dados do Pub Med, ScienceDirect, Elsevier, Capes, Bireme, Scielo, Medline, BBO e Lilacs. Resultado: A mucosite é classificada clinicamente em cinco graus, indo desde eritema a ulcerações, sem ou com comprometimento da alimentação e deglutição. Tratamentos múltiplos de ordem local e sistêmica, preventivos e curativos são descritos na literatura, dependendo do grau de complicação apresentado. Entretanto, os cuidados com a higiene bucal, antes, durante e após o tratamento radio ou quimioterápico, são essenciais para minimizar esses efeitos adversos. Conclusão: O cirurgião-dentista tem um papel importante na equipe multidisciplinar no sentido de prevenir, diagnosticar e tratar pacientes submetidos a tratamentos antineoplásico. Ele deve iniciar um programa de higiene oral e ser um consolidador destas medidas durante e após a químio e radioterapia, seguindo um protocolo proposto e viável de aplicação.
