Dynamics of circulation of multi-drug resistant bacteria in pediatric intensive care units of Rio de Janeiro

Resumo: | Background and Objectives: The circulation of multiresistant bacteria between pediatric intensive care units is a reality and it is necessary to know the profi le of the most prevalent microorganisms to establish effective control measures. The aim of this study is to describe the profi le of imported multiresistant bacteria (MRB) in a pediatric intensive care unit in Rio de Janeiro, according to the geographic region the hospitals. Methods: Nasal and rectal swabs were collected from all patients who remained for more than 12 hours in other units, and were admitted at the four pediatric intensive care units of Prontobaby. Results: Between August 2009 and December 2011. Sixty BMR were detected, of which 19 caused infection and 41, colonizations. Gram-negative infections accounted for 68.4% and 60.8% of the colonizations. Most hospitals (21/33- 63.6%) were located in the city of Rio de Janeiro and those located in the north and west regions of the city had 25 of the 60 (41.6%) BMR. Klebsiella pneumoniae ESBL was the most commonly isolated bacteria in infections, while Escherichia coli ESBL was the most frequent in Gram-negative colonizations. Conclusion: We conclude that the imported Gram-negative MRB were more prevalent in both infections and colonizations. The north and west regions of the city had the most cases of MRB. KEYWORDS: children. infections. bacteria.Justifi cativa e Objetivos: A circulação de bactérias multirresistentes entre unidades de terapia intensiva pediátricas é uma realidade, sendo necessário conhecer o perfi l dos microrganismos mais prevalentes para estabelecer medidas de controle efi cazes. O objetivo deste artigo é descrever o perfi l das bactérias multirresistentes (BMR) importadas em uma unidade de terapia intensiva pediátrica do Rio de Janeiro, de acordo com a região geográfi ca dos hospitais. Métodos: Foi realizada coleta de swab nasal e retal em todos os pacientes que permaneceram mais que 12 horas em outras unidades, admitidos nas 4 unidades de terapia intensiva pediátrica do Prontobaby. Resultados: Entre agosto de 2009 e dezembro de 2011, detectamos 60 BMR sendo 19 causadoras de infecção e 41 colonizações. Bactérias Gram negativas representaram 68,4% das infecções e 60,8% das colonizações. A maior parte dos hospitais (21/33- 63,6%) estava localizada na cidade do Rio de Janeiro e aqueles localizados na zona norte e oeste concentraram 25 das 60 (41,6%) BMR. Klebsiella pneumoniae ESBL foi a bactéria mais comumente isolada em infecções enquanto Escherichia coli E SBL f oi a mais frequente em colonizações por Gram negativos. Conclusão: Concluímos que as BMR gram negativas importadas foram mais prevalentes tanto em infecções quanto em colonizações.
