Tratamento de erupção passiva alterada com cirurgia plástica periodontal: relato de caso clínico

Gummy smile constitutes a relatively frequent aesthetic alteration characterized by excessive exhibition of the gums during smiling movements of the upper lip. Altered Passive Eruption (APE) is a clinical situation produced by excessive gum overlapping over the enamel limits, resulting in a short clinical crown appearance. The purpose of this article is to report the treatment of a case of APE associated with gingivitis diagnosed in a white female patient, 18 years-old, at the Dental School of Araçatuba/UNESP. The patient complained of difficulties to perform oral hygiene procedures and gingival bleeding. She also wanted to increase the clinical crown of the anterior teeth. After obtaining the medical history, clinical and radiographic exams were performed and an APE associated with gingivitis was diagnosed in the anterior region of the upper and lower arches, with pseudo recession in some areas. The initial periodontal therapy related to the cause was performed. At the reevaluation, it was observed that the APE was still an issue and a periodontal plastic surgery was planned to correct it. A gingivoplasty was performed to restore the normal anatomy of the gingiva in order to facilitate the oral hygiene procedures and to improve esthetics. The clinical success of the case was verified in several postoperative follow-ups. APE is a factor that frequently contributes to the presence of a gummy smile, and it can be easily be corrected by periodontal surgery. In addition, the patient's motivation to maintain oral hygiene is a fundamental requirement for the maintenance of periodontal health
