Influência do nível socioeconômico no desempenho funcional de crianças com paralisia cerebral

Resumo: The term cerebral palsy (CP) refers to a group of progressive chronic conditions of childhood, which affects the central nervous system in the maturation phase, causing abnormalities in the coordination of movement. Among the risk factors that increase the likelihood of deficits in motor development, is the socioeconomic status (SES) of the family. Despite evidence that the environment can interfere with child development information on the influence of socioeconomic status in the performance of functional skills and independence to carry out routine tasks of children with cerebral palsy are not well defined. The aim of this study was to investigate the influence of socioeconomic status on performance in children with cerebral palsy. This was a cross-sectional study with 49 children with cerebral palsy, aged between 3 and 8 years old, and their caregivers. The instruments used were: Pediatric Evaluation of Disability Inventory (PEDI), GMFCS (Gross Motor Function Classification System), Economic Classification of Brazil and an identification sheet with information about the child and the caregiver. The results showed that children with CP were as serious as most influential in the SES. Although these children of low SES have the skill to self-care significantly lower than that of children with high SES, the independence of that performance was similar in the two economic classes. Children with impairment varied similar performance in the skill of social function. However, the independence of the social function, children with CP moderately low SES are more dependent than children with high SES. When compared to children with high SES and low, regardless of motor impairment, it was observed that children with high SES and low skill have the mobility and independence in mobility also similar, but when it comes to self-care and social function of both the functional skill as independence, the low SES children performed significantly lower than children with high SES. The social function was the best demonstrated the differences between caregivers of children with low education and caregivers of high school. It was concluded that children with CP may be influenced by SES, especially the direct influence of the caregiver, which she can both receive adequate protection or excessive, or be exposed to risks to their development. The social risk of belonging to a family's economic status affects the most disadvantaged children with severe impairment, those with the greatest biological risk. For the families of high socioeconomic class, when it comes to children with serious, seem to be overprotective in relation to self-care, giving aid over the daily tasks. Regardless of the motor impairment of children with CP, families of high socioeconomic class, offer more opportunities to develop skills and independence in the areas of self-care and social function of families of low SES. The implementation of appropriate public policies of maternal-child care can reduce the negative impact of SES and low education in the functional performance of children with CP.

Dentre os fatores de risco que aumentam a probabilidade de déficits no desenvolvimento motor, está o nível socioeconômico da família. Apesar das evidências de que o ambiente possa interferir no desenvolvimento infantil, informações sobre a influência do nível socioeconômico no desempenho das habilidades funcionais e na assistência do cuidador para realizar tarefas da rotina diária da criança com paralisia cerebral não estão bem definidas. O objetivo deste estudo foi investigar a influência do nível socioeconômico no desempenho funcional de crianças com paralisia cerebral. Tratou-se de estudo transversal com 49 crianças com paralisia cerebral, idade entre 3 e 8 anos, e seus respectivos cuidadores. Os instrumentos utilizados foram: Inventário de Avaliação Pediátrica de Incapacidade (PEDI), GMFCS (Sistema de Classificação da Função Motora Grossa), Critério de Classificação Econômica do Brasil e uma ficha de identificação com informações sobre a criança e o cuidador. Os resultados apontaram que crianças com PC grave foram as que mais tiveram influência do NSE. Apesar de essas crianças de NSE baixo terem habilidade de autocuidado significantemente inferior à das crianças de NSE alto, a assistência do cuidador desse mesmo desempenho foi semelhante nas duas classes econômicas. Crianças com comprometimento variado têm desempenho semelhante na habilidade da função social. Entretanto, na assistência do cuidador na função social, as crianças com PC moderada de NSE baixo são mais dependentes do que crianças com NSE alto. Quando se compararam crianças de NSE alto e baixo, independentemente do comprometimento motor, observou-se que crianças com NSE alto e baixo têm habilidade na mobilidade e assistência do cuidador também na mobilidade semelhantes, porém quando se trata do autocuidado e função social tanto da habilidade funcional quanto na assistência do cuidador, as crianças de NSE baixo apresentaram desempenho significativamente inferior às crianças de NSE alto. A função social foi a que melhor demonstrou as diferenças entre crianças com cuidadores de baixa escolaridade e cuidadores de alta escolaridade. Conclui-se que a criança com PC pode sofrer influência do NSE, sobretudo influência direta do cuidador, do qual ela pode tanto receber proteção adequada ou excessiva, quanto conviver com riscos para o seu desenvolvimento. O risco social de pertencer a uma família de classe econômica desfavorecida afeta mais crianças com comprometimento grave, ou seja, crianças com maior risco biológico. Já as famílias de classe econômica alta, quando se trata de crianças com comprometimento grave, parecem ser superprotetoras em relação ao autocuidado, dando ajuda excessiva nas tarefas diárias. Independentemente do comprometimento motor das crianças com PC, famílias de classe econômica alta dão mais oportunidades para o desenvolvimento de habilidades e a independência nas áreas de autocuidado e função social do que famílias de NSE baixo.
