The meaning of fidelity and Aids prevention strategies among married men

Resumo: | OBJECTIVE: To describe the meaning of fidelity and condom use among married men and to suggest HIV risk reduction and Aids prevention strategies. METHODS: Ten men residing in Americanópolis, a neighborhood in the city of São Paulo, Americanópolis, were interviewed using a semi-structured questionnaire. The sample was drawn through contacts established in a preliminary research among married women. Interview analyses focused on the social build up of masculinity and its meanings. RESULTS: As to fidelity, the study group perceives as natural for a man not to have his wife as his only sex partner. Fidelity is understood as respecting their wives, which is translated into the use of condoms in extra-marital relationships that are seen as associated with disease risk and vulnerability. Sexual intercourse with their wives is perceived as risky because it is based on love and companionship, and the use of condoms is only for contraception especially when other birth control methods cannot be used. CONCLUSIONS: In the study, men revealed a very limited perception of their vulnerability to Aids. Promoting the use of condoms in extra-marital relationships meets this society's definitions of masculinity. There is a need of a more efficient strategy in the short run for risk reduction among men and their wives, and the development of other strategies that incorporate structural changes of genre relations. Condom use should be promoted among married men as a contraceptive method, as well as a form of fidelity and family protection. |

OBJETIVOS: Descrever as representações sobre fidelidade e os usos do preservativo por homens casados e sugerir estratégias de redução de risco e prevenção da Aids. MÉTODOS: Dez homens casados, moradores de Americanópolis, bairro da periferia do Município de São Paulo, foram entrevistados, a partir de um roteiro semi-estruturado. Foram contatados por "bola de neve" a partir do informante chave (preferencial) identificado em pesquisa preliminar com mulheres casadas. As entrevistas foram analisadas segundo o enfoque da construção cultural da masculinidade e das representações sociais. RESULTADOS: Observou-se que o grupo estudado tinha um entendimento de que a fidelidade tornava natural para o gênero masculino não ter a esposa como única parceira sexual. Entendiam fidelidade como o respeito à parceira e o conseqüente uso da camisinha nas relações extraconjugais, que estão associadas ao risco de infecção, e que as relações sexuais com a esposa não eram perigosas porque estavam baseadas no amor e no companheirismo. Nelas, o uso do preservativo era bem-vindo apenas quando o objetivo era a contracepção, especialmente quando outro método não podia ser utilizado. CONCLUSÕES: Os homens estudados tiveram uma percepção bastante limitada da sua vulnerabilidade para Aids. Incentivar que aconteçam as relações sexuais extraconjugais com o uso do preservativo permite levar em conta as definições culturais para masculinidade. Propõe-se uma estratégia mais aceitável de redução do risco para esse grupo (e suas esposas) a curto prazo, sem deixar de pensar numa mudança mais estrutural nas relações de gênero. A camisinha deve ser estimulada também entre homens casados como forma de contracepção, associada à fidelidade e proteção da família. Universidade de São Paulo.
