Notificação da perda auditiva induzida por ruído ocupacional: percepção e conhecimento dos profissionais de saúde da atenção primária

Resumo: | Occupational diseases are generally underreported in Brazil. Among others, is noise-induced hearing loss (NIHL), considered an occupational disease notifiable, Ministry of Health through the Computerized Disease Surveillance System (SINAN) and the Ministry of Labor and Employment through the Accident Report Work (CAT). Purpose of this study was to analyze the perception and knowledge of health professionals of Primary Health Care (PHC) in the city of Curitiba, on the compulsory notification of NIHL. Methods: A quantitative and qualitative approach in cross-sectional study carried out in three steps: 1) analysis of the time series of reported cases of NIHL in SINAN database from 2007 to 2014; 2) application of semi-structured questionnaire to the health of the PHC network professionals; 3) press conference with nurses, speech therapists and doctors from Curitiba PHC network. Results: The number of NIHL notifications during the study period was 44 cases, males represented (90.9%), the age range was from 50 to 64 years, completed elementary school represented (25%) and the contract the formal labor (52.3%). The CAT was issued to (43.2%) of the cases notified in SINAN. Workers Industry Manual are the most exposed to noise (36.4%), and the Manufacturing Industry represented the highest number of reported cases (41%). In the statement of semi-structured questionnaire, 48 professionals in network PHC answered the questionnaire said they feel prepared to identify health problems related to the employee (68.7%), 33 subjects answered that the act of making guidance to ensure the user is more comprehensive care important than doing the obligatory notification, in the statement of the perceived knowledge of NIHL characteristics (56.2%) said yes, (43.7%) feels able to identify cases of NIHL and (52.1% ) do not conduct the notification to the SINAN. The outcome of the third step, the health professionals report difficulties: the correct way to perform the notification of NIHL for not understanding the notification flow, and the legal implications that can generate with the notification for the professional, difficulty request audiometric testing, lack of management support. Conclusion: the results found, noted that the health professionals of the PHC network, have knowledge about NIHL, but is afraid to report suspected cases, does not know the notification flow and feel little support from management regarding the health program worker in Primary Health Care Network in Curitiba. |

Entre outras, está a Perda Auditiva Induzida por Ruído (PAIR), considerada uma doença ocupacional passível de notificação tanto pelo Ministério da Saúde via Sistema Informatizado de Agravos de Notificação (SINAN), bem como, pelo Ministério do Trabalho e Emprego via Comunicação de Acidente de Trabalho (CAT). Objetivo do presente estudo foi analisar a percepção e conhecimento do profissional de saúde da Atenção Primária à Saúde (APS) do município de Curitiba, sobre a notificação compulsória da PAIR. Métodos: utilizou-se uma abordagem quanti-qualitativas no estudo transversal realizado em três etapas: 1) análise da série histórica dos casos notificados de PAIR no banco de dados do SINAN no período de 2007 a 2014; 2) aplicação de questionário semiestruturado para profissionais da saúde da rede APS; 3) entrevista coletiva com enfermeiros, fonoaudiólogos e médicos da rede de APS de Curitiba. Resultados: O número de notificações de PAIR no período estudado foi de 44 casos, o sexo masculino representou (90,9%), a faixa de idade foi entre 50 a 64 anos, o ensino fundamental completo representou (25%) e o contrato de trabalho formal (52,3%). A CAT foi emitida para (43,2%) dos casos notificados no SINAN. Os trabalhadores da Indústria Manual são os mais expostos ao ruído (36,4%), e a Indústria de Transformação representou o maior número de casos notificados (41%). No resultado do questionário semiestruturado, 48 profissionais na rede APS responderam o questionário, disseram que sentem preparado para identificar problemas de saúde relacionados ao trabalhador (68.7%), 33 sujeitos responderam que o ato de fazer orientação para garantir o cuidado integral do usuário é mais importante do que fazer a notificação compulsória, no demonstrativo da percepção do conhecimento das características da PAIR (56,2%) disseram que sim, (43,7%) sente-se apto para identificar os casos de PAIR e (52,1%) não realizam a notificação para o SINAN. No resultado da terceira etapa, os profissionais de saúde relatam dificuldades: na correta maneira de realizar a notificação da PAIR por não entenderem o fluxo de notificação, e as implicações legais que podem gerar com a notificação para o profissional, dificuldade em solicitar exame audiométrico, falta de apoio da gestão. Results: The number of NIHL notifications during the study period was 44 cases, males represented (90.9%), the age range was from 50 to 64 years, completed elementary school represented (25%) and the contract the formal labor (52.3%). The CAT was issued to (43.2%) of the cases notified in SINAN. Workers Industry Manual are the most exposed to noise (36.4%), and the Manufacturing Industry represented the highest number of reported cases (41%). In the statement of semi-structured questionnaire, 48 professionals in network PHC answered the questionnaire said they feel prepared to identify health problems related to the employee (68.7%), 33 subjects answered that the act of making guidance to ensure the user is more comprehensive care important than doing the obligatory notification, in the statement of the perceived knowledge of NIHL characteristics (56.2%) said yes, (43.7%) feels able to identify cases of NIHL and (52.1% ) do not conduct the notification to the SINAN. The outcome of the third step, the health professionals report difficulties: the correct way to perform the notification of NIHL for not understanding the notification flow, and the legal implications that can generate with the notification for the professional, difficulty request audiometric testing, lack of management support. Conclusion: the results found, noted that the health professionals of the PHC network, have knowledge about NIHL, but is afraid to report suspected cases, does not know the notification flow and feel little support from management regarding the health program worker in Primary Health Care Network in Curitiba. |
