Postural Evaluation of Patients Subjected to Modified Radical Mastectomy by Means of Computer Photogrammetry

Resumo: Introduction: Modified radical mastectomy surgery can result in changes in posture of women stricken with breast cancer. In order to more precisely develop a physiotherapeutic regimen, computer photogrammetry is being widely used as a diagnostic tool, especially for postural evaluation. Objective: The aim of this study was to analyze the posture of patients who have undergone modified radical mastectomy by means of computer photogrammetry. Methods: This is a cross-sectional study, conducted at the Physiotherapy Center of the Cancer Hospital of Pernambuco, from March to May 2007. The sample consisted of 22 women who had undergone modified radical mastectomy for treatment of breast cancer, with ages from 37 to 60 years, who had not undergone postural physiotherapeutic evaluation in the pre-surgery phase. The images were captured in four protocols of postural analysis which were accomplished in the anterior and posterior views, as well as in the left and right lateral views. Subsequently, the images were analyzed using the software SAPO, version 0.67. In the comparative analyses, the Student’s-t test was used, with significance level of p<0.05 used for all the variables and performed with the SPSS program, version 8.0. Results: Several postural changes were observed, but the main changes related to surgical intervention were the forward disposition of the head (25.84±11.33, p=0.001) and protrusion of shoulder abduction (29.79±7.64, p=0.001). Conclusion: Women who undergo surgical treatments such as modified radical mastectomy for treatment of breast cancer can develop postural changes and the most common ones related to the surgery are forward disposition of the head and protrusion of the shoulder abduction.

Para conducir de modo más preciso la conducta fisioterápica, la fotogrametría computadorizada se ha difundido ampliamente como herramienta de diagnóstico, especialmente en evaluaciones posturales. Propósito: El propósito de ese estudio fue evaluar la postura de los pacientes que se sometieron a cirugía de mastectomía radical modificada por fotogrametría computarizada. Métodos: Es un estudio transverso, hecho en el sector de Fisioterapia del Hospital de Cáncer de Pernambuco, en el período de marzo hasta mayo de 2007. La muestra fue constituida por 22 mujeres sometidas a la cirugía de mastectomía radical cambiada para tratamiento de cáncer de mama, con edades entre 37 y 60 años, sin evaluación de postura fisioterápica en la fase prequirúrgica. Las imágenes fueron capturadas en cuatro protocolos de análisis postural y obtenidas en las vistas anterior y posterior, laterales derecha e izquierda. Posteriormente, las imágenes fueron analizadas vía software SAPO versión 0.67. En los análisis comparativos fue utilizado el test t-Student, siendo p<0,05 para todas las variables y realizadas en el programa SPSS, versión 8.0. Resultados: Fueron observadas diversas alteraciones posturales, sin embargo las principales alteraciones posturales encontradas se relacionan a la parte anterior de la cabeza (25,84±11,33; p=0,001) y a la protrusión del hombro homolateral (29,79±7,64; p=0,001) la intervención quirúrgica. Conclusión: Las mujeres que realizan el tratamiento quirúrgico del tipo mastectomía radical modificada para tratar el cáncer de mama pueden presentar cambios posturales y los principales están relacionados con la disposición anterior de la cabeza y la protrusión del hombro homolateral a la cirugía.  Introdução: A cirurgia de mastectomia radical modificada pode acarretar alterações na postura das mulheres acometidas por câncer de mama. Para guiar mais precisamente a conduta fisioterapêutica, a fotogrametria computadorizada vem sendo amplamente difundida como recurso diagnóstico, especialmente em avaliações posturais. Objetivo: O objetivo deste estudo foi avaliar a postura de pacientes submetidas à cirurgia de mastectomia radical modificada através da fotogrametria computadorizada. Métodos: Trata-se de um estudo do tipo transversal, realizado no setor de Fisioterapia do Hospital de Câncer de Pernambuco, no período de março a maio de 2007. A amostra foi constituída por 22 mulheres submetidas à cirurgia de mastectomia radical modificada para tratamento do câncer de mama, com idades entre 37 e 60 anos, sem avaliação postural fisioterapêutica na fase pré-cirúrgica.
