Prevalence and Clinical Profile of the Phantom Breast Syndrome: Integrative Review

Resumo: Introduction: The breast phantom syndrome consists of the feeling of persistence of the breast after its withdrawal and encompasses both painful and non-painful sensations. Although it is reported in population studies, its clinical aspects as well as its prevalence remain unknown. Objective: To evaluate the profile of population-based research conducted either in Brazil or worldwide that analyzed the clinical features as well as the incidence or prevalence of breast phantom. Method: An integrative Literature Review from 2002-2011, carried out by means of bibliographic search in the PubMed, LILACS and SciELO databases. Articles that investigated clinical characteristics as well as the prevalence of breast phantom syndrome in Brazil and throughout the world were chosen. Results: In the selected period, four prospective studies and 10 cross-sectional studies that met the selection criteria were identified and established for this literature review. A wide range of assessment methods was found, showing a lack of standardization. It is observed that although several studies evaluate and describe the development of pain and postoperative morbidities, a few sought to delimit the characteristics of breast phantom, becoming a secondary subject in the process of dealing with the mastectomy. Conclusion: It is necessary to emphasize the importance of further exploration of the Breast Phantom Syndrome with post-surgical morbidity of breast cancer, so that certain actions be directed to its best knowledge and research.

A pesar de que sea relatada en estudios de población, sus aspectos clínicos, así como su prevalencia permanecen desconocidos. Objetivo: Trazar el perfil de los estudios de población realizados tanto en Brasil cuanto alrededor del mundo que investigaron las características clínicas y la prevalencia, así como la incidencia o prevalencia de la sensación de la mama fantasma, ya sea doloroso o no. Método: Revisión Integradora de la literatura en el período entre 2002 y 2011, llevada a cabo a través de términos específicos en las bases de datos PubMed, LILACS y SciELO. Han sido rescatados artículos que investigaron las características clínicas, así como la prevalencia de la síndrome de la mama fantasma en Brasil y en el mundo. Resultados: En el período seleccionado, se identificaron cuatro estudios prospectivos y 10 transversales que cumplieron los criterios de selección establecidos para esta revisión bibliográfica. Se encontró una amplia gama de métodos de evaluación, evidenciando falta de estandarización. Se observa que aunque varios estudios evaluaron y describieron el desarrollo de cuadros álgidos y las morbilidades postoperatorias, pocos intentaron delimitar las características de la mama fantasma, convirtiéndose em un tema secundario en el proceso del tratamiento con mastectomía. Conclusión: Es necesario destacar la importância de más exploración de la síndrome de la mama fantasma como morbilidad postquirúrgica del cáncer de mama, para que determinadas acciones sean dirigidas a su mejor conocimiento y investigación.  Introdução: A síndrome da mama fantasma consiste na sensação de persistência da mama após sua retirada e engloba tanto as sensações não dolorosas quanto as dolorosas. Embora seja relatada em estudos populacionais, os seus aspectos clínicos assim como sua prevalência permanecem desconhecidos. Objetivo: Traçar o perfil das pesquisas populacionais realizadas tanto no Brasil quanto ao redor do mundo que investigaram as características clínicas assim como a incidência ou prevalência da sensação da mama fantasma, seja essa dolorosa ou não. Método: Revisão integrativa da literatura no período de 2002 a 2011, realizada por meio de termos específicos nos bancos de dados PubMed, LILACS e SciELO. Foram resgatados artigos que investigaram as características clínicas, assim como a prevalência da síndrome da mama fantasma no Brasil e no Mundo. Resultados: No período selecionado, foram identificados quatro estudos prospectivos e dez transversais que preenchiam os critérios de seleção estabelecidos para essa revisão bibliográfica. Uma vasta gama de métodos de avaliação foi encontrada, evidenciando uma ausência de padronização. Observou-se que, apesar de diversos estudos avaliarem e descreverem o desenvolvimento de quadros álgicos e morbidades pós-operatórias, poucos buscaram delimitar as características da mama fantasma, tornando-se um assunto secundário no processo de lidar com a mastectomia.
