Elderly with Alzheimer's disease: the care and the knowledge of the familial caregiver

Resumo: This is a descriptive qualitative study carried out in a Referral Center for Alzheimer's disease in the city of Curitiba - Paraná. The objective was to reveal the knowledge and the care system of family members that care for elderly with Alzheimer's disease. Information was collected at the patients'home from 14 caregivers via semi-structured interviews. The following domains emerged from the analysis: protection measures and stimulus as consequence of a non-acceptance of the disease; the disease symptoms cause fatigue in the caregiver; modifications in family life as a result of Alzheimer's disease; awareness of the disease by personal experience and by the media. Knowledge of family caregivers about Alzheimer's disease is limited to information provided by the media in brochures distributed to the population and to common sense knowledge. This contributes to the employment of non-recommended practices and increases the caregiver responsibility. Care is generally accepted and developed uncritically by caregivers that are imposed by family tradition. It is understood that providing appropriate information is essential for professionals who want to develop the caregiver's autonomy and decision-making when caring for the elderly. If they are informed about the disease and its evolution caregivers will have the opportunity to plan the care for the elderly and to delay the grievances characteristics of the disease and thus minimize the impact on their lives.

El objetivo fue revelar el sistema de conocimiento y cuidado de cuidadores familiares de ancianos portadores de la enfermedad de Alzheimer. Las informaciones fueron recogidas en el domicilio con 14 cuidadores por medio de entrevistas semiestructuradas. De los análisis, surgieron los temas: cuidados de protección y estímulo en consecuencia de la no aceptación de la enfermedad; los síntomas de la enfermedad causan desgaste para el cuidador; cambios en la vida familiar debido a la enfermedad de Alzheimer; el conocimiento de la enfermedad por medio de las experiencias propias y de los medios de comunicación. El conocimiento de los cuidadores familiares acerca de la enfermedad de Alzheimer está limitado a las informaciones suministradas por los medios de comunicación, en folders distribuidos a la población y al del sentido común lo cual contribuye a la manutención de cuidados poco recomendables y potencia la sobrecarga del cuidador. En general, el cuidado se acepta y los cuidadores lo realizan de modo acrítico, impuesto por la tradición familiar. Se entiende que el suministrar información adecuada es una actividad indispensable para el profesional que desea desarrollar la autonomía y tomar decisiones al cuidar al anciano. Los cuidadores, cuando informados acerca de la enfermedad y de su evolución, tendrán la oportunidad de planear su trabajo, administrar los agravios de la enfermedad y, en consecuencia, minimizar el impacto sobre sus vidas.Trata-se de estudo qualitativo-descritivo, realizado num Centro de Referência em Doença de Alzheimer da Cidade de Curitiba-PR. O objetivo foi revelar o sistema de conhecimento e cuidado de cuidadores familiares de idosos portadores da doença de Alzheimer. A coleta das informações ocorreu em domicílio, com 14 cuidadores, por meio de entrevistas semiestruturadas. Das análises emergiram os domínios: cuidados de proteção e estímulo em consequência da não aceitação da doença; os sintomas da doença geram desgaste para o cuidador; modificações na vida familiar decorrentes da doença de Alzheimer; o conhecimento da doença por meio das experiências próprias e da mídia. O conhecimento dos cuidadores familiares a respeito da doença de Alzheimer limita-se às informações fornecidas pela mídia, pelos folders distribuídos à população e pelo conhecimento do senso comum, o que contribui para a manutenção de cuidados pouco recomendáveis e potencializa a sobrecarga do cuidador. O cuidado é geralmente aceito e desenvolvido pelos cuidadores de modo acrítico, imposto pela tradição familiar. Entende-se que fornecer informações apropriadas é uma atividade indispensável para o profissional que deseja desenvolver no cuidador a autonomia e a tomada de decisão no momento de prestar o cuidado ao idoso.
