Resumo: The brazilian mental health policy project is based on thepolicy adopted by other countries in the past. However, these countriesare actually suffering the consequences of several difficulties andlimitations of this community mental health policy, such as: lack ofsufficient and adequate community centers, ideological struggles amongmental health professionals, lack of coordination among the services, lack of preparation of the community to accept the patients, lack of permanentcaring to the patient. The consequences of these difficulties have been theobstruction of the emmergencies and the psychiatric units of the generalhospitals, overcrowded community centers which do not differ very muchfrom the psychiatric hospitals, rejection of the patients by the society andan increase in the number of rehospitalizations. A general discussion ofthe brazilian project, based on the lessons from other countries isproposed.

A general discussion ofthe brazilian project, based on the lessons from other countries isproposed.RESUMO - O projeto de política de saúde mental do Brasil apresenta características semelhantes às de vários outros países. Estes, porém, já convivem com os erros e limites desta política e já apontam seus mitos e dificuldades: falta de apoio financeiro suficiente para criar serviços comunitários adequados, falta de formação, preparação e engajamento da comunidade no acolhimento ao doente mental, dificuldade de coordenação dos diversos setores dos serviços oferecidos, divergências ideológicas entre os trabalhadores da saúde mental, membros das equipes interdisciplinares, falta de tolerância com os sub-grupos mais deficientes de pacientes, falta de um atendimento permanente a cada paciente. Esta configuração tem levado a resultados difíceis: o aumento dos casos tratados fora dos hospitais é acompanhado de aumento também dos casos tratados em instituição, numa taxa maior que a do crescimento da população. Muitas instituições na comunidade diferem pouco dos hospitais psiquiátricos. Observa-se um problema de obstrução das unidades psiquiátricas e das emergências de hospitais gerais, que têm se tornado a porta de entrada do paciente. Observa-se um aumento de re-hospitalizações de um contingente de pacientes jovens, pobres, desempregados, itinerantes, que acabam sendo recolhidos pela igreja ou transitam nas ruas ou prisões.
