Salivary proteins and early childhood caries: literature review

Resumo: | Introduction: Dental caries is the most prevalent chronic disease in childhood, being a significant public health problem. Saliva is considered one of the most important host factors and an essential biological modulator that controls the speed and progression of caries. Objective: To perform a literature review on early childhood caries (ECC) and its possible relationship with salivary proteins and peptides. Methods: An electronic search was performed in the Scopus, SciELO and PubMed Medline databases, applying the combination of the terms "early childhood caries", "saliva", "peptides" and "proteins". This research included studies that aim to relate salivary proteins and peptides and dental caries, individuals aged 0-71 months, studies published in the English language, randomized trials and clinical trials that demonstrate the relationship between early childhood caries and salivary proteins. Studies conducted with patients with congenital and / or chromosomal syndromes, systemic disorders or motor disabilities, as well as studies in animals, review studies, letters to the editor or case reports were not included in this review. Results: Of the 48 studies identified, 13 were selected for this review. Most studies have shown an association between salivary proteins and peptides and the presence or absence of caries. Six studies showed a high level of quality, while the rest of the studies were classified as of moderate quality. Conclusion: Although the association between salivary proteins and ECC is established, more evidence is needed for salivary biomarkers to be considered predictors of ECC risk. |

La saliva se considera uno de los factores del huésped más importantes y un modulador biológico esencial que controla la velocidad y progresión de la caries. Objetivo: Realizar una revisión de la literatura sobre caries infantil temprana (CPI) y su posible relación con proteínas y péptidos salivales. Métodos: Se realizó una búsqueda electrónica en las bases de datos Scopus, SciELO y PubMed Medline, aplicando la combinación de los términos "caries infantil temprana", "saliva", "péptidos" y "proteínas". Esta investigación incluyó estudios que tienen como objetivo relacionar proteínas y péptidos salivales y caries dental, individuos de 0 a 71 meses, estudios publicados en el idioma inglés, ensayos aleatorizados y ensayos clínicos que demuestran la relación entre la caries infantil temprana y las proteínas salivales. Los estudios realizados con pacientes con síndromes congénitos y/o cromosómicos, trastornos sistémicos o discapacidades motoras, así como estudios en animales, estudios de revisión, cartas al editor o informes de casos no se incluyeron en esta revisión. Resultados: De los 48 estudios identificados, 13 fueron seleccionados para esta revisión. La mayoría de los estudios demostraron una asociación entre proteínas y péptidos salivales y la presencia o ausencia de caries. Seis estudios mostraron un alto nivel de calidad, mientras que el resto de los estudios se clasificaron como calidad moderada. Conclusión: Aunque se establece la asociación entre proteínas salivales e IPC, se necesita más evidencia para que los biomarcadores salivales se consideren predictores de riesgo de CPI.Introdução: A cárie dentária é a doença crônica mais prevalente na infância, sendo um problema significativo de saúde pública. A saliva é considerada um dos fatores hospedeiros mais importantes e um modulador biológico essencial que controla a velocidade e progressão da cárie. Objetivo: Realizar uma revisão da literatura sobre cárie na primeira infância (CPI) e sua possível relação com proteínas e peptídeos salivares. Métodos: Uma busca eletrônica foi realizada nas bases de dados Scopus, SciELO e PubMed Medline, aplicando a combinação dos termos "cárie na primeira infância", "saliva", "peptídeos" e "proteínas". Foram incluídos nesta pesquisa estudos que visam relacionar proteínas e peptídeos salivares e cárie dentária, indivíduos com idade entre 0-71 meses, estudos publicados na língua inglesa, ensaios randomizados e ensaios clínicos que demonstram a relação entre cárie na primeira infância e proteínas salivares. Estudos realizados com pacientes com síndromes congênitas e/ou cromossômicas, distúrbios sistêmicos ou deficiências motoras, bem como estudos em animais, estudos de revisão, cartas ao editor ou relatos de caso não foram incluídos nesta revisão. Resultados: Dos 48 estudos identificados, 13 foram selecionados para esta revisão. A maioria dos estudos mostrou associação entre proteínas e peptídeos salivares e a presença ou ausência de cárie. Seis estudos mostraram um alto nível de qualidade, enquanto o restante dos estudos foi classificado como de qualidade moderada.
