Effect of nonsurgical periodontal treatment in the control of rheumatoid arthritis

Resumo: Periodontitis is a non communicable chronic inflammatory pathology of microbial etiology, which results in loss of periodontal attachment. Rheumatoid arthritis -RA- is a chronic systemic autoimmune disease. Periodontitis and RA share similar humoral and cellular immune responses and a common immunogenic profile; both conditions can bidirectionally influence the systemic burden of inflammation. Nonsurgical periodontal treatment -NSPT- for periodontitis may have beneficial effects on RA disease activity. Objective: to identify the available scientific evidence on the impact of NSPT in periodontitis on the control of RA disease activity. Methodology: narrative review of the literature; 10 publications in English -from 2009 to 2021- were selected from the PUBMED, COCHRANE and WILEY scientific databases; information on the impact of NSPT on periodontitis in patients with RA was identified; the results were expressed in text and a summary table. Results: The NSPT of periodontitis had an impact on the decrease in the activity of the RA disease. The most used disease activity index in patients with RA was the DAS28. NSPT improved the clinical parameters of periodontitis. For all this, dental care and periodontal treatment are necessary in the multidisciplinary management of patients with RA.

La artritis reumatoide -AR- es una enfermedad autoinmune sistémica crónica. La periodontitis y la AR comparten respuestas inmunes humorales y celulares similares y un perfil inmunogénico común; ambas afecciones pueden influir de manera bidireccional en la carga sistémica de la inflamación. El tratamiento periodontal no quirúrgico -TPNQ- de la periodontitis puede tener efectos beneficiosos sobre la actividad de la enfermedad de la AR. Objetivo: identificar la evidencia científica disponible sobre el impacto del TPNQ de la periodontitis sobre el control de la actividad de la enfermedad de la AR. Metodología: revisión narrativa de la literatura; se seleccionaron 10 publicaciones en inglés -desde 2009 hasta 2021-, de las bases de datos científicas PUBMED, COCHRANE y WILEY; se identificó información del impacto de la TPNQ de la periodontitis en pacientes con AR; los resultados se expresaron en texto y una tabla resumen. Resultados: el TPNQ de la periodontitis tuvo un impacto sobre la disminución de la actividad de la enfermedad de la AR. El índice de actividad de la enfermedad más usado en el paciente con AR, fue el DAS28. El TPNQ mejoró los parámetros clínicos de la periodontitis. Por todo esto, es necesaria la atención odontológica y el tratamiento periodontal en el manejo multidisciplinario del paciente con AR.A periodontite é uma patologia inflamatória crônica não transmissível de etiologia microbiana, que resulta em perda de inserção periodontal. O TPNC melhorou os parâmetros clínicos da periodontite.
