Resumo: Objectives: To Map and describe the activities and risks of a surgical process in a specialized hospital, and propose strategies for patient safety through a risk management proposal. Methodology: Qualitative research, in the form of action research, carried out with 12 professionals working in a surgical center of a specialized hospital in northwestern São Paulo, from May to June 2017. Brainstorming tools were used to understand the internal flow, Business Process Model and Notation and Supplier, Input, Process, Output and Customer for process mapping, National Health Agency's Risk Matrix for risk stratification and improvement proposals, the methodology proposed by the Institute of Healthcare Improvement. Results: In the mapping, the operating room was divided into admission; operating room and anesthetic recovery, which resulted in 13 processes and 42 activities described. It was possible to observe relevant interactions with administrative and support processes. 42 risk situations were identified; 31.8% were classified as medium risk, 43.1% high and 25.0% extreme, which generated 87 proposals for interventions to improve patient safety. Conclusion: Mapping the activities and risks of surgical processes provides the manager with tools for decision making; above all, the adoption of corrective strategies for each of the risk points in the flow of patient care.

Metodología: Investigación cualitativa, en forma de investigación acción, realizada con 12 profesionales que laboran en un centro quirúrgico de un hospital especializado del noroeste de São Paulo, de mayo a junio de 2017. Se utilizaron herramientas de lluvia de ideas para comprender el flujo interno, Modelo de Procesos de Negocio y Notación y Proveedor, Entrada, Proceso, Salida y Cliente para el mapeo de procesos, Matriz de Riesgos de la Agencia Nacional de Salud para la estratificación de riesgos y propuestas de mejora, la metodología propuesta por el Instituto de Mejoramiento de la Salud. Resultados: En el mapeo, el quirófano se dividió en ingreso; quirófano y recuperación anestésica, lo que resultó en 13 procesos y 42 actividades descritas. Se pudo observar interacciones relevantes con procesos administrativos y de apoyo. Se identificaron 42 situaciones de riesgo; El 31,8% fueron clasificados como de riesgo medio, el 43,1% alto y el 25,0% extremo, lo que generó 87 propuestas de intervenciones para mejorar la seguridad del paciente. Conclusión: el mapeo de las actividades y los riesgos de los procesos quirúrgicos proporciona al gerente herramientas para la toma de decisiones; sobre todo, la adopción de estrategias correctivas para cada uno de los puntos de riesgo en el flujo de atención al paciente.Objetivos: Mapear e descrever as atividades e riscos de um processo cirúrgico em um hospital especializado, e propor estratégias para a segurança do paciente por meio de uma proposta de gestão de risco. Metodologia: Pesquisa qualitativa, na modalidade de pesquisa-ação, realizado com 12 profissionais atuantes em um centro cirúrgico de um hospital especializado do noroeste paulista, no período de maio a junho de 2017. Foram utilizadas as ferramentas brainstorming para conhecimento do fluxo interno, Business Process Model and Notation e o Supplier, Input, Process, Output e Cliente para mapeamento do processo, Matriz de Risco da Agência Nacional de Saúde Suplementar para a estratificação dos riscos e para as propostas de melhorias, utilizou-se a metodologia proposta pelo Institute of Healtcare Improvement.  Resultados: No mapeamento, o centro cirúrgico foi dividido em admissão; sala cirúrgica e recuperação anestésica, que resultaram em 13 processos e 42 atividades descritas. Foi possível observar interações relevantes com processos administrativos e de apoio. Foram identificadas 42 situações de risco; sendo que 31,8% foram classificadas como risco médio, 43,1% alto e 25,0% extremo, que geraram 87 propostas de intervenções para a melhoria da segurança do paciente.
