Summary: Phantom pain is actually a sensation known and observed after amputations, being like pain of a surgical remotion of a member or portion this. The pain may start immediately after of the amputation or sometimes it appears weeks, months and even years later. It feels in precises points of phantom pain . Occuring generally in 70% of amputated. Some authors describe that the appearance of phantom pain may be associated with the experience pain before amputation, denominated of memory of the pain. It is also a predisposed factor to appearance of painfull phantom. Several theories and supposition get involved as attempts to explain of appearance of phantom pain, being that neuromatrix published by Melzack, is the most satisfactory. TheNeuromatrix proposes that our brain is constituted to network spread the loops between thalamus and cortex, as well as, cortex and limbic system, at initially determined genetically and later altered by sensory stimulus. The lost stimulus to limb or to body stimulate the neuromatrix produced a abnormal standard sign, as heat, burn, cramp and tingling sensation. It isobserved that intensity the phantom limb pain experimented by amputed correlate with extensions of the reorganization functional the cortex somatosensory. The phantom limb pain can't be explained satisfatory for any mechanism single, such as irritation of a peripheric nerve, abnormal activity simpathic or psycopathology problems, all some form contribute to appealment and perpetuation of pain.

A dor pode começar imediatamente depois da amputação ou, às vezes, aparece semanas, meses e mesmo anos mais tarde. Sente-se em pontos precisos do membro fantasma, ocorrendo geralmente em 70% dos amputados. Alguns autores descrevem que o aparecimento da dor fantasma pode estar associada com a dor experimentada antes da amputação, denominada de memória da dor. Várias teorias e suposições envolvem as tentativas de explicar o aparecimento da dor fantasma, das quais a neuromatrix, publicada porMelzack em 1990, é a mais satisfatória. A Neuromatrix propõe que o nosso cérebro é formado por uma rede difundida de neurônios consistindo de voltas entre o tálamo e o córtex, assim como, o córtex e o sistema límbico; sendo inicialmente determinada geneticamente e mais tardealterada pelos estímulos sensorials. A perda de estímulo para o membro ou para o corpo ativa a neuromatrix a produzir uma assinatura padrão anormal, como calor, queimação, cãibra e uma sensação de formigamento. E observado que a intensidade da dor do membro fantasma experimentado pelos amputados correlaciona-se com a extensão da reorganização funcional do córtex somatosensorial. A dor do membro fantasma não pode ser explicada satisfatoriamente por qualquer mecanismo único, tal como irritação de um nervo periférico, atividade anormal do simpático ou problemas psicopatológicos, já que todos, de alguma forma, contribuem para o aparecimento e perpetuação da dor.Universidade de São Paulo.
