Necessidade de suporte ventilatório em prematuros de um hospital do sul do Brasil

Resumo: Introduction: Prematurity in newborns is a condition that is associated with worse hospital outcomes when compared to full-term birth. The determinant gestational age of a preterm newborn (PTNB) is less than 37 weeks, related to some results that generate the anticipation of delivery. Due to the importance of early recognition of PTNBs who will need some type of ventilatory support, the objective of this study was to analyze prognostic indicators related to the use of oxygen therapy, CPAP and invasive mechanical ventilation in PTNB. Methods: This is a retrospective cohort study. The sample consisted of premature newborns at the SOCIMED Hospital in Tubarão Santa Catarina. For inclusion, the following criteria were adopted: newborns of both sexes, preterm born with less than 37 weeks of gestation in the period from 01/01/2018 to 12/31/2018. Exclusion criteria were medical records with incomplete data, newborns transferred to another hospital. Electronic medical records were used in the Tasy PhilipsTM system of the SOCIMED Hospital in Tubarão / SC for data collection. Results: 90 medical records of PTNB and their maternal history were analyzed. Of these, 81 were cesarean deliveries, 18 twin births, 9 were born in normal birth, where 50 of these were first pregnancy, 45 (50%) of whom were boys. Statistical significance was observed for the mother's lowest age, lowest gestational age, lowest birth weight, APGAR <8 and death, and were associated with patients who needed MV compared to those who did not need oxygen support. Conclusion: Prematurity can rarely be avoided, due to maternal and child complications, but the essential thing is to have a qualified team and technologies available to promptly attend to unforeseen events that may happen after birth.

A idade gestacional determinante de um recém-nascido pré termo (RNPT) é inferior a 37 semanas, relacionado a alguns resultantes que geram a antecipação do parto. Devido à importância no reconhecimento precoce dos RNPTs que irão necessitar de algum tipo de suporte ventilatório, o objetivo deste trabalho foi analisar indicadores prognósticos relacionados ao uso de oxigenoterapia, CPAP e ventilação mecânica invasiva no RNPT. Métodos: Trata-se de um estudo do tipo coorte retrospectivo. A amostra foi composta por recém-nascidos prematuros no Hospital SOCIMED em Tubarão Santa Catarina. Para a inclusão foram adotados os seguintes critérios: recém-nascidos de ambos os sexos, pré termo nascidos com menos de 37 semanas de gestação no período de 01/01/2018 a 31/12/2018. Os de exclusão foram prontuários com dados incompletos, recém-nascidos transferidos para outro hospital. Foram utilizados prontuários eletrônicos no sistema Tasy PhilipsTM do Hospital SOCIMED em Tubarão/SC para a coleta de dados. Resultados: Foram analisados 90 prontuários de RNPT e seus antecedentes maternos. Destes, 81 foram partos cesarianas, 18 gemelares, nascidos de parto normal foram 9, onde 50 destes eram de primeira gestação, sendo 45 (50%) meninos. Foi observada significância estatística a idade da mãe mais baixa, a menor idade gestacional, o menor peso ao nascer, APGAR<8 e óbito, e foram associados a pacientes que tiveram necessidade de VM comparados com que não precisaram de suporte de oxigênio.
