Desafios da adesão intrarradicular: revisão de literatura

Resumo: Current scientific evidence points to more reliable results for the cementation of intraradicular retainers with glass ionomer and self-adhesive resin cements when compared with adhesive resin. Achieving stable adhesion to intraradicular dentin, especially apical, is an enormous clinical challenge due to the influence of several factors. In addition to the limited access to light inside the duct, the technical conditions and characteristics of the substrate and the cementing agent are also determining factors of the result. Inside the duct, the control of humidity, quantity and evaporation of the solvent present in the applied adhesive and the use of oxidizing agents for intraradicular cleaning can compromise the complete polymerization of the resin cement. In addition to these factors, there is the incompatibility of simplified adhesive systems, which accompany most resin cements, with chemical and dual polymerization resins. In addition, the excessive permeability of the simplified adhesives and the acidity of their final layer make these adhesives behave as permeable membranes after polymerization, allowing the transudation of water through the adhesive layer and increasing the likelihood of long-term degradation. Thus, the present study aims to carry out a narrative review of the literature with regard to the adhesion of intraradicular retainers and to elucidate the main challenges, interferences and supporting factors with regard to internal root anatomy, filling materials, cementing agents and techniques that involve restorations using intracanal pins. Cements.A evidência científica atual aponta resultados mais confiáveis para a cimentação de retentores intrarradiculares com ionômero de vidro e cimentos resinosos autoadesivos quando comparados com os resinosos adesivos. A obtenção de uma adesão estável à dentina intrarradicular, principalmente apical, se constitui um enorme desafio clínico devido à influência de diversos fatores. Além do limitado acesso à luz no interior do conduto, as condições técnicas e características do substrato e do agente cimentante também são fatores determinantes do resultado. No interior do conduto, o controle da umidade, da quantidade e evaporação do solvente presente no adesivo aplicado e o uso de agentes oxidantes de limpeza intrarradicular podem comprometer a polimerização completa do cimento resinoso. Somam-se a estes fatores a incompatibilidade dos sistemas adesivos simplificados, que acompanham a maioria dos cimentos resinosos, com as resinas de polimerização química e dual. Além disso, a excessiva permeabilidade dos adesivos simplificados e a acidez de sua camada final fazem com que tais adesivos se comportem como membranas permeáveis após a polimerização, permitindo a transudação de água pela camada adesiva e aumentando a probabilidade de degradação em longo prazo. Assim, o presente estudo tem como objetivo realizar uma revisão narrativa da literatura no que diz respeito a adesão de retentores intrarradiculares e elucidar os principais desafios, interferências e fatores coadjuvantes no que se refere a anatomia radicular interna, materiais obturadores, agentes cimentantes e técnicas que envolvam restaurações utilizando pinos intracanal.
