Resumo: | Paracoccidioidomycosis (PCM) is a systemic granulomatous infection, caused by the fungus Paracoccidioides brasiliensis, which predominates in Latin America, mainly in Brazil followed by Colombia, Venezuela and Argentina. In Brazil, the incidence is higher in the South, Southeast and Midwest. In Paraná, the disease is endemic among the population of the rural area, and because it affects individuals in its production phase, it is an important point within public health. Estimates of annual incidence in Brazil range from 0.71 cases to 3.70 cases per 100 thousand inhabitants. However, the incidence and prevalence of paracoccidioidomycosis is underestimated due to the fact that it does not have a compulsory notification, thus hindering the detailed characterization of the current situation in the country regarding this pathology. Due to the numerous stomatological manifestations found, it was believed until recently that the oropharynx was the entrance door of the fungus. Therefore, the knowledge of PCM is of great stomatological interest, as the first signs and symptoms of the disease will lead the patient to the dentist or otolaryngologist. The oral cavity and the neck region are important areas for the manifestation of the disease, since the oral mucosa provides the basis for the saprophytic life of fungi. Understanding this disease is of great dental interest because, in its manifestations, oral lesions contribute to its clinical characterization and meet the needs of dentists to participate in the diagnosis and treatment of this ringworm. The present project was a retrospective study of a clinical case report of a 55-year-old male patient with a manifestation of a moriform lesion in the oral cavity due to inoculation of the fungus in a pre-existing lesion through the straw cigarette contaminated by the fungus Paracoccidioides brasiliensis. The patient had dental care and an early diagnosis of systemic fungal infection and was referred to the Infectologist. Conclusion: Premature diagnosis of PCM is essential in order to avoid, by establishing early therapy, the most mutilating forms of this disease. So, the role of the dentist in the diagnosis and treatment of paracoccidioidomycosis is crucial, since patients seek care due to the stomatological manifestations of this disease. |

No Brasil, a incidência é maior na Região Sul, Sudeste e Centro-Oeste. No Paraná, a doença é endêmica entre a população da zona rural e, por acometer indivíduos em sua fase de produção, é um ponto importante dentro da saúde pública. Estimativas de incidência anual no Brasil variam de 0,71 casos a 3,70 casos por 100 mil habitantes. No entanto, a incidência e prevalência da paracoccidioidomicose é subestimada devido a não ter notificação compulsória, dificultando, assim, a caracterização detalhada da situação atual do país frente à essa patologia. Devido às inúmeras manifestações estomatológicas encontradas, acreditava-se até recentemente ser a orofaringe a porta de entrada do fungo. Por isso, o conhecimento da PCM apresenta grande interesse estomatológico, pois os primeiros sinais e sintomas da doença irão conduzir o paciente ao dentista ou ao otorrinolaringologista. A cavidade oral e a região do pescoço são áreas importantes da manifestação da doença, pois a mucosa bucal fornece a base para a vida saprofítica dos fungos. Sendo assim, o entendimento dessa doença tem grande interesse odontológico, pois, em suas manifestações, as lesões bucais contribuem para sua caracterização clínica e atendem às necessidades dos cirurgiões-dentistas em participar do diagnóstico e tratamento dessa micose. O presente projeto foi um estudo retrospectivo de relato de caso clínico de um paciente do sexo masculino de 55 anos de idade com manifestação de lesão moriforme em cavidade bucal devido a inoculação do fungo em lesão pré-existente, por meio do cigarro de palha contaminado pelo fungo Paracoccidioides brasiliensis. O paciente teve atendimento odontológico e diagnóstico precoce da infecção fúngica sistêmica e foi encaminhado para o Infectologista. Conclusão: O diagnóstico prematuro da PCM é fundamental a fim de evitar, pelo estabelecimento de uma terapêutica precoce, as formas mais mutilantes dessa doença.
