The Psychiatry Specialty Medical Outpatient Clinic at the beginning of the COVID-19 pandemic

Resumo: Introduction: No country was adequately prepared for the COVID-19 pandemic. According to the World Health Organization (WHO), 93% of countries across the world have experienced some kind of interruption in mental health services. AME Psychiatry used information from international and national health authorities to make decisions, in order to adapt daily routines and rules in the unit. Objective: The objective of this study is to describe the adjustments carried out by a public service specialized in mental health at the beginning of the pandemic, to maintain a safe health care assistance against the risks of COVID-19’s contamination, based on available information. Additionally, it will provide the profile data of patients seen before and after the beginning of the pandemic. Method: Cross-sectional study. Adjustments made based on management reports, procedure manuals, meeting minutes and training presentations. Sample of 10 120 patients selected by convenience, who attended at least one individual appointment with a psychiatrist between May 2019 and December 2020. This time period was determined in order to obtain a comparison of two groups, pre-pandemic (May 2019 to February 2020) and post-pandemic (March to December 2020), with the same time amount of 10 months. Results: The adaptations made enabled the AME Psychiatry to maintain its health care services and to compare the profile of patients who started the treatment before the pandemic with those who started it after. There was no change in terms of patients’ sex in the two analyzed periods, with 65% of them being women and 35% men. There was a reduction of 26% regarding child and teenager patients (aged 0 to 19) and of 14% for the elderly (aged 60 and above), and a 7% raise for adults (aged 20 to 59). Conclusion: Although the study shows COVID-19’s direct and indirect factors as predisposing to the development of mental disorders, we cannot affirm that these changes in the diagnostic profile of AME Psychiatry patients after the beginning of the pandemic are related to COVID-19. On the other hand, the publication of findings from a public outpatient clinic specialized in mental health that maintained consultations at the beginning and during the pandemic, is of great value to support other researchers in this area.

AME Psychiatry utilizó información de autoridades sanitarias nacionales e internacionales para la toma de decisiones, con el fin de adecuar las rutinas y normas diarias en la unidad. Objetivo: El objetivo de este estudio es describir los ajustes realizados por un servicio público especializado en salud mental al início de la pandemia, para mantener una asistencia sanitaria segura frente a los riesgos de contagio por COVID-19, con base en la información disponible. Adicionalmente, brindará los datos de perfil de los pacientes atendidos antes y después del início de la pandemia. Método: Estudio transversal. Ajustes realizados en base a informes de gestión, manuales de procedimientos, actas de reuniones y presentaciones de capacitación. Muestra de 10 120 pacientes seleccionados por conveniencia, que asistieron al menos a una cita individual con un médico psiquiatra entre mayo de 2019 y diciembre de 2020. Este período de tiempo se determinó con el fin de obtener una comparación de dos grupos, prepandemia (mayo de 2019 a febrero de 2020) y pospandemia (marzo a diciembre de 2020), con el mismo tiempo de 10 meses. Resultados: Las adaptaciones realizadas permitieron a la AME Psiquiatría mantener sus servicios de atención en salud y comparar el perfil de los pacientes que iniciaron el tratamiento antes de la pandemia con los que lo iniciaron después. No hubo cambio en cuanto al sexo de los pacientes en los dos períodos analizados, siendo el 65% mujeres y el 35% hombres. Hubo una reducción del 26% en pacientes niños y adolescentes (0 a 19 años) y del 14% en ancianos (60 años y más), y un aumento del 7% en adultos (20 a 59 años). Conclusión: Si bien el estudio muestra los factores directos e indirectos del COVID-19 como predisponentes para el desarrollo de trastornos mentales, no podemos afirmar que estos cambios en el perfil diagnóstico de los pacientes de AME Psiquiatría luego del início de la pandemia estén relacionados con el COVID-19. Segundo a Organização Mundial de Saúde (OMS), 93% dos países em todo o mundo tiveram algum tipo de interrupção nos serviços de saúde mental. O AME Psiquiatria utilizou a informações de autoridades de saúde internacionais e nacionais para a tomada de decisões na adaptação de normas e rotinas na unidade. Objetivo: O objetivo deste estudo é descrever as adaptações realizadas por um serviço público especializado em saúde mental no início da pandemia para manter a assistência segura frente aos riscos de contaminação pela COVID-19 com base nas informações disponíveis. Assim como fornecer os dados do perfil dos pacientes atendidos pré e pós início da pandemia. Método: Estudo transversal. Adaptações descritas com base em relatórios de gestão, manuais de procedimentos, atas de reunião e apresentações de treinamentos. Amostra de 10.120 pacientes selecionados por conveniência que frequentaram pelo menos um atendimento individual com psiquiatra entre maio de 2019 a dezembro de 2020. Este período foi escolhido obter um comparativo de dois grupos pré (maio 2019 a fevereiro de 2020) e pós pandemia (março a dezembro de 2020) com o mesmo tempo de 10 meses. Resultados: As adaptações realizadas possibilitaram a manutenção dos atendimentos no AME Psiquiatria e a comparação do perfil dos pacientes que iniciaram tratamento antes da pandemia versus após início da pandemia. Não houve mudança em relação ao sexo nos dois períodos analisados, sendo que 65% das pessoas eram mulheres e 35% homens. Houve redução em 26% de crianças e adolescentes (faixa etária de 0 a 19 anos) e de 14% de idosos (60 anos ou mais), com aumento de 7% de adultos na faixa etária de 20 a 59 anos. Conclusão: Apesar dos estudos apontarem fatores diretos e indiretos da COVID-19 como predisponentes para o desenvolvimento de transtornos mentais, não podemos afirmar que esta mudança de perfil diagnóstico dos pacientes do AME Psiquiatria pós início da pandemia tenha relação com a COVID-19.
