General psychiatric management (GPM) for borderline personality disorder: a generalist model for Latin America

Resumo: For many years, borderline personality disorder (BPD) has been regarded as an untreatable mental health condition. However, several effective evidence-based treatments developed in the last decades, such as dialectical behavior therapy (DBT), mentalization-based treatment (MBT) and transference focused psychotherapy (TFP), proved otherwise. The problem from a public health perspective is that their availability is limited. In this context of increased need for trained clinicians, general psychiatric management (GPM) and other generalist approaches emerged. This article reviews GPM’s major foundations and framework that guide non-specialized clinicians to structure a treatment based on medicalization of the disorder, case management and supportive psychotherapeutic interventions. We also illustrate the approach with a case vignette and finally present newer developments, such as integration with DBT and GPM for adolescents (GPM-A).

Aun así, diversos tratamientos efectivos basados en la evidencia fueron desarrollados en las últimas décadas, como, Terapia Dialéctica Conductual (DBT), Tratamiento Basado en la Mentalización (MBT) y Psicoterapia Centrada en la Transferencia (TFP), los cuales probaron lo contrario. El problema desde el punto de vista de la salud pública es que la disponibilidad de estas intervenciones es limitada. En este contexto de gran demanda de clínicos entrenados, el Manejo Psiquiátrico General (GPM) y otros abordajes generalistas surgieron. Este artículo revisa la estructura y los principales fundamentos del GPM que orientan a los clínicos no especialistas a planear un tratamiento basado en la medicalización del trastorno, en el gerenciamiento del caso y en intervenciones psicoterapéuticas de apoyo. Nosotros también ilustramos el abordaje con un ejemplo de caso y finalmente, presentamos los desarrollos más recientes, como la integración con el DBT y el GPM para adolescentes (GPM-A).Por muitos anos, o Transtorno de Personalidade Borderline (TPB) foi considerado como uma condição de saúde mental intratável. No entanto, diversos tratamentos desenvolvidos nas últimas décadas, como a Terapia Comportamental Dialética (DBT), o Tratamento Baseado na Mentalização (MBT) e a Psicoterapia Focada na Transferência (TFP) provaram o contrário. O problema do ponto de vista de saúde pública é que a disponibilidade dessas intervenções é limitada. Nesse contexto de alta demanda de profissionais treinados, o Bom Manejo Clínico (GPM) e outras abordagens generalistas surgiram. Este artigo revisa a estrutura e os principais fundamentos do GPM que orientam os clínicos não especialistas a planejar um tratamento baseado na medicalização do transtorno, no gerenciamento de caso e em intervenções psicoterapêuticas de suporte.
