Bipolar phenotypes and phenocopies and their variation in the life cycle

Resumo: | Introduction: Rare studies address bipolar disorder (BD) from a developmental psychopathology perspective and, therefore, little is known about how the symptoms of this disease present themselves over time, at different stages of human development. Methodology: In this narrative review we will describe the unusual presentations of TB (“clinical disguises” or phenocopies) throughout the life cycle and their implications for differential diagnosis and prognosis. Results: The main psychiatric disguises linked to TB throughout the life cycle are described. The semiological difficulties and, consequently, the diagnostic errors associated with TB, which in turn reflect on the proper functioning and increased costs of public health. Conclusions: Other mental disorders and neurological diseases may be falsely recognized as BD, as the diagnosis is still supported by clinical data. Translational research may in the future enable the identification of biomarkers and improve differential diagnosis. It is also important to consider the clinical stages of the disease in well-characterized samples of young people at high risk during diagnosis. |

Metodología: En esta revisión narrativa describiremos las presentaciones inusuales de la tuberculosis (“disfraces clínicos” o fenocopias) a lo largo del ciclo de vida y sus implicaciones para el diagnóstico diferencial y el pronóstico. Resultados: Se describen los principales disfraces psiquiátricos vinculados a la tuberculosis a lo largo del ciclo vital. Las dificultades semiológicas y, en consecuencia, los errores diagnósticos asociados a la tuberculosis, que a su vez se reflejan en el buen funcionamiento y aumento de los costos de la salud pública. Conclusiones: Otros trastornos mentales y enfermedades neurológicas pueden reconocerse erróneamente como TB, ya que el diagnóstico aún está respaldado por datos clínicos. La investigación traslacional puede permitir en el futuro la identificación de biomarcadores y mejorar el diagnóstico diferencial. También es importante considerar las etapas clínicas de la enfermedad en muestras bien caracterizadas de jóvenes con alto riesgo durante el diagnóstico.Introdução: Raros trabalhos abordam o transtorno bipolar (TB) de uma perspectiva da psicopatologia do desenvolvimento e, portanto, ainda pouco se sabe sobre como os sintomas dessa doença se apresentam ao longo do tempo, em diferentes estágios de desenvolvimento humano. Metodologia: Nesta revisão narrativa descreveremos as apresentações não-usuais do TB (“disfarces clínicos” ou fenocópias) ao longo do ciclo da vida e suas implicações para o diagnóstico diferencial e prognóstico. Resultados: São descritos os principais disfarces psiquiátricos ligados ao TB ao longo do ciclo de vida. As dificuldades semiológicas e, consequentemente, os erros diagnósticos associados ao TB, que por sua vez refletem no bom funcionamento e aumento de custos da saúde pública. Conclusões: Outros transtornos mentais e doenças neurológicas podem ser falsamente reconhecidos como TB, visto que o diagnóstico ainda é apoiado em dados clínicos. A pesquisa translacional pode no futuro propiciar a identificação de biomarcadores e melhorar o diagnóstico diferencial.
