Principles and ethical decision models in liaison and consulting psychiatry

Resumo: | In everyday clinical practice, Liaison and Consultancy Psychiatry (PLC) is often called upon to participate in the clarification and resolution of situations with challenging clinical and ethical implications. However, the training of professionals dedicated to this subspecialty does not imply a differentiated training in Clinical Ethics compared to other physicians. This article intends to explore the various principles and models of ethical decision underlying the activity in PLC. A narrative review of the relevant literature available on the PubMed and Google Scholar platforms was conducted, using the keywords “ethics”, “consultation-liaison psychiatry'', “psychosomatic medicine” and “end of life care”, completed with the exploration of the references considered relevant. from other sources Five models of decision-making in the face of ethical dilemmas are presented, starting from a historical point of view. The ethical principles involved in clinical practice are often in conflict, which is why there is a need for models that systematize the analysis of ethical dilemmas and simplify their resolution. The structured approach to Ethics during the psychiatrist's formative period can provide knowledge and proficiency in the application of ethical principles and decision-making models, fundamental for an impartial and effective practice. |

Sin embargo, la formación de los profesionales dedicados a esta subespecialidad no implica una formación diferenciada en Ética Clínica frente a otros médicos. Este artículo pretende explorar los diversos principios y modelos de decisión ética que subyacen a la actividad en PLC. Se realizó una revisión narrativa de la literatura relevante disponible en las plataformas PubMed y Google Scholar, utilizando las palabras clave “ética”, “psiquiatría de enlace de consulta”, “medicina psicosomática” y “cuidados al final de la vida”, completada con la exploración de las referencias consideradas relevante de otras fuentes Se presentan cinco modelos de toma de decisiones ante dilemas éticos, partiendo de un punto de vista histórico. Los principios éticos involucrados en la práctica clínica a menudo están en conflicto, por lo que existe la necesidad de modelos que sistematicen el análisis de los dilemas éticos y simplifiquen su resolución. El abordaje estructurado de la Ética durante el período de formación del psiquiatra puede proporcionar conocimiento y competencia en la aplicación de principios éticos y modelos de toma de decisiones, fundamentales para una práctica imparcial y eficaz.Na prática clínica quotidiana, a Psiquiatria de Ligação e Consultoria (PLC) é frequentemente chamada a participar no esclarecimento e resolução de situações com implicações clínicas e éticas desafiantes. No entanto, a formação dos profissionais dedicados a esta subespecialidade não pressupõe um treino diferenciado em Ética Clínica comparativamente com os restantes médicos. Neste artigo pretende-se explorar os diversos princípios e modelos de decisão ética subjacentes à atividade em PLC. Foi conduzida uma revisão narrativa da literatura relevante disponível nas plataformas PubMed e Google Acadêmico, utilizando as palavras-chave “ethics”, "consultation-liaison psychiatry'', “psychosomatic medicine” e “end of life care”, completada com a exploração das referências consideradas relevantes noutras fontes. Apresentam-se cinco modelos de tomada de decisão perante dilemas éticos, partindo de um ponto de vista histórico.Os princípios éticos envolvidos na prática clínica encontram-se frequentemente em confronto, pelo que há necessidade de modelos que sistematizem a análise dos dilemas éticos e simplifiquem a sua resolução.
